Morton neuroma: sonographic evaluation.

Abstract: One hundred consecutive patients with symptoms suggestive of Morton neuroma were examined with sonography, and 134 intermetatarsal masses were demonstrated. Forty-five patients underwent surgical exploration, which revealed Morton neuromas. The typical sonographic appearance is that of an ovoid, hypoechoic mass oriented parallel to the long axis of the metatarsals. Most masses were between the second and third or third and fourth metatarsals and seemed to produce symptoms when reaching a diameter of 5 mm.

“…Intense tenderness elicited by pressure with the transducer over the suspected neuroma supports the diagnosis. In the appropriate clinical setting, US has a reported sensitivity of 95%-100% (30,31,36) for detection of Morton neuromas, with a specificity of 83% and accuracy of 95% (31). Similar results have been reported for MR imaging, which has demonstrated a sensitivity of 87%, specificity of 100%, and accuracy of 89% (37).…”

“…US evaluation can be performing by placing the probe on the dorsal (30,31) or plantar (32) aspect of the foot. For the dorsal approach, the toes are plantar flexed and manual pressure in the affected web space is applied on the plantar side.…”

“…Not infrequently, the diagnosis or the exact interdigital space involved is uncertain. Sonography is then of value in demonstrating a solid, hypoechoic soft tissue mass between the metatarsal heads, [22][23][24][25][26] although disappointing results have been reported. 27 I use a plantar sonographic approach in cases of suspected Morton's neuroma.…”

“…Injuries smaller than five millimeters may be difficult to visualize at the ultrasound test (1) . Ultrasound results prove that masses were found at the first space in 8% of the cases, at the second space in 44%, at the third in 46%, and at the fourth space in 2% of the cases (4) . Magnetic resonance is an imaging test that certainly shows the neuroma, its characteristics and size (5,6) .…”
