Mortality Risk for Acute Cholangitis (MAC): a risk prediction model for in-hospital mortality in patients with acute cholangitis

Abstract: BackgroundAcute cholangitis is a life-threatening bacterial infection of the biliary tract. Main focus of this study was to create a useful risk prediction model that helps physicians to assign patients with acute cholangitis into different management groups.Methods981 cholangitis episodes from 810 patients were analysed retrospectively at a German tertiary center.ResultsOut of eleven investigated statistical models fit to 22 predictors, the Random Forest model achieved the best (cross-)validated performance t… Show more

“…As a disadvantage of the TG13 severity grading criteria, it is pointed out that each prognostic factor is equally weighted, and the highest OR was reported to be observed for mental confusion . Similar to this study, our results showed that respiratory dysfunction had the highest OR, followed by neurological dysfunction, in univariate analysis.…”

“…Regarding the outcomes among patients with acute cholangitis, our study provides updated and comprehensive data for overall mortality and complications among patients with acute cholangitis. The previously available data have been only from single or few institutions and may not have reflected true mortality for a long period of time . Our data demonstrated the mortalities increased along with the severity grades and this reflects validity of the TG13 severity assessment .…”

“…However, despite newer biliary drainage techniques, increasing antimicrobial resistance, and increasing number of elderly patients with multiple medical problems in many areas of the world, clinically updated and comprehensive epidemiological data on patients with acute cholangitis have been very limited . Recent available data on patients with acute cholangitis have been on relatively small cohorts of patients from single institutions or a small number of institutions . In Europe, prospective studies on patients with complicated intra‐abdominal infections have been performed and published .…”
