Background It remains unsettled whether alcoholic cirrhosis is a risk factor for myocardial infarction (MI). Methods We used data from nationwide healthcare registries to study all Danes diagnosed with alcoholic cirrhosis in 1996‐2014, and five controls were matched to each of them on gender and age. We excluded everyone with ischaemic heart disease and used Cox regression to estimate the incidence rate ratio of MI adjusted for potential cardiovascular confounders. Further, we described the MI‐risk with non‐MI death as a competing risk. Results We included 22 867 patients (67% men) with a median age of 57 years. During the first year of follow‐up, their incidence rate ratio of MI was increased to 1.24 (95% CI: 0.94‐1.62), driven by the effect among women (2.13, 95% CI: 1.17‐3.87) and those with most severe cirrhosis (1.32, 95% CI: 0.91‐1.90). After the first year, the overall incidence rate ratio fell to (0.89, 95% CI: 0.76‐1.05). Patients were more likely to die from non‐MI causes (33.7% vs 1.0%), which protected them against MI. The overall 1‐year MI‐risk was similar in patients and controls: 0.38% (95% CI: 0.30‐0.47%) vs 0.34% (95% CI: 0.31‐0.38%). After five years of follow‐up, male patients had lower MI‐risk than their controls, whereas women with cirrhosis had an increased MI‐risk throughout follow‐up. Conclusions The incidence rate of MI was increased the first year following a diagnosis of alcoholic cirrhosis, in particular in women and those with most severe liver disease. Due to the competing risk of non‐MI mortality, the MI‐risk was not increased.

BackgroundCerebral stroke is a severe and frequent condition that requires rapid and reliable diagnosis. If administered shortly after the first symptoms manifest themselves, IV thrombolysis has been shown to increase the functional prognosis by restoring brain reperfusion. However, a better understanding of the pathophysiology of stroke should help to identify potential new therapeutic targets. Stroke is known to induce an inflammatory brain reaction that involves overexpression of the 18-kDa translocator protein (TSPO) in glial cells and infiltrated leukocytes, which can be visualised by positron emission tomography (PET). We aimed to evaluate post-stroke neuroinflammation using the PET TSPO radioligand 18 F-DPA-714.MethodsNine patients underwent 18 F-DPA-714 PET and magnetic resonance imaging (MRI) between 8 and 18 days after the ictus. Co-registration of MRI and PET images was used to define three volumes of interest (VOIs): core infarction, contralateral region, and cerebellum ipsilateral to the stroke lesion. Time activity curves were obtained from each VOI, and ratios of mean and maximum activities between the VOIs were calculated.ResultsWe observed an increased uptake of 18 F-DPA-714 co-localised with the infarct tissue and extension beyond the region corresponding to the damage in the blood brain barrier. No correlation was identified between 18 F-DPA-714 uptake and infarct volume. 18 F-DPA-714 uptake in ischemic lesion (mainly associated with TSPO expression in the infarct area and in the surrounding neighbourhood) slowly decreased from 10 min pi to the end of the PET acquisition, remaining higher than that in both contralateral region and ipsilateral cerebellum.ConclusionOur results show that 18 F-DPA-714 uptake after acute ischemia is mainly associated with TSPO expression in the infarct area and in the surrounding neighbourhood. We also demonstrated that the kinetics of 18 F-DPA-714 differs in injured tissue compared to normal tissue. Therefore, 18 F-DPA-714 may be useful in assessing the extent of neuroinflammation associated with acute stroke and could also help to predict clinical outcomes and functional recovery, as well as to assess therapeutic strategies, such as the use of neuroprotective/anti-inflammatory drugs.

Objective: To compare the performance of Cockcroft-Gault and Modification of Diet in Renal Disease (MDRD) equations in estimating kidney function in CKD patients with diabetes and hypertension. Methods: This study retrospectively reviewed medical records in Hospital Kajang. The GFR was calculated using Cockcroft-Gault and MDRD equations. Kappa Measure of Agreement was used to check the consistency of CKD staging. Wilcoxon signed-ranked tests and Bland-Altman plots were used to determine the difference of both equations. Spearman correlation was used to determine the correlation between blood pressure and blood sugar levels with eGFR. Results: Data pertaining to a total of 81 patients were extracted. Results showed 22% of the patients were staged differently (Kappa value = 0.644 [P<0.001]) and the majority of them moved down one CKD stage when MDRD equation was used instead of Cockcroft-Gault equation. Wilcoxon signed rank test demonstrated there was a significant difference (P<0.001) in eGFR using CandG and MDRD in patients with diabetes and hypertension. Furthermore, the mean difference observed was 3.78±5.56 [P<0.001]), where the Cockcroft-Gault equation measured 3.78 units higher than MDRD equation. However, the relationship between blood sugar and blood pressure with eGFR were not significant. Conclusion: There was a significant difference between Cockcroft-Gault and MDRD equations in estimating kidney function CKD patients with diabetes and hypertension.
