Mortality and Clinical Outcomes in HIV-Infected Children on Antiretroviral Therapy in Malawi, Lesotho, and Swaziland

Abstract: Despite challenges associated with pediatric ART in developing countries, low mortality and good treatment outcomes can be achieved. However, outcomes are worse in younger patients and those with advanced disease at the time of ART initiation, highlighting the importance of early diagnosis and treatment.

“…Median CD4 + T cell percentage increased from 3.16 (IQR 1-18) % to 11.0 (IQR 2-32) %, whereas median CD4 + absolute cell count increased from 9.5 (IQR 3-176) cell/mm 3 to 419.5 (IQR 202-1428) cell/mm 3 . Baseline level of CD4 + cell count in our study was very low compared with other studies [15,[19][20][21][22], but this result was not much different from baseline levels from 8 sites in Asia [14]. After ART initiation, CD4…”

“…11 out of 83 (13.3%) children were ambulatory, one was bedridden, while the rest of the study subjects (85.5%) were in working functional status. The signs, symptoms and opportunistic infections were almost similar to those reported earlier from developed and developing countries, including India 11,17,22,23,24 , although small variations can be attributed to the wide spectrum of disease all over the world. Management of these infections should be ensured in the ART clinics.…”

“…Similar observations were made by number of researchers 13,18,19,20,21 who reported a significant proportion of children (started on HAART) to be suffering from moderate or severe form of disease (31% -76%). 11 out of 83 (13.3%) children were ambulatory, one was bedridden, while the rest of the study subjects (85.5%) were in working functional status.…”

“…It was estimated that earlier access to ART could have prevented 25% of HIV related death [8,9]. However, access to ART for children was only 12% among children under 15 years of age [1].…”
