Objective: To investigate the existence of a causal association between orthopedic treatment and the occurrence of in hospital death. Methods: 338 patients with proximal femoral fracture were evaluated, of whom 27 faced in hospital death. Patients who faced in hospital death (case group) were compared to patients who did not (control group) regarding exposure to risk factors prior to injury and factors related to orthopedic treatment. Results: The factors related to higher in hospital mortality rate were: male sex (case group: 52%, control: 26%; p = 0.005), lower Parker’s score (case group: 5.0 points, control: 6.2; p = 0.048), delirium on admission (case group: 26%, control: 10%; p = 0.011); delirium developed during hospitalization (case group: 77%, control: 35%; p <0.001), and time until surgery (13.3 days, 9.1; p = 0.049). Conclusion: The in hospital mortality rate of patients with proximal femoral fracture was 8%, and the main associated risk factors were male sex, reduced Parker’s score, delirium diagnosed on hospital admission or developed during hospitalization, and time until surgery. Objetivo: analisar os fatores que interferem no desfecho de internação de idosos com fratura defêmur.Método: transversal exploratório documental, população de idosos (≥60 anos) no período de 2015a 2017, do norte do Paraná. Foram realizadas análises bivariadas de associações e distribuição defrequência.Resultados: dos 189 prontuários analisados, 132 (69,9%) idosos desenvolveram morbidades noambiente hospitalar. A ocorrência de infecção urinária, sepse, pneumonia, lesão por pressão ea presença de duas ou mais comorbidades foram associadas ao desfecho óbito e ao tempo dehospitalização superior a 10 dias p<0,001.Conclusão: morbidades preexistentes e hospitalares em idosos com fratura de fêmur elevaram ahospitalização pelo tempo superior a 10 dias e com desfecho de óbito. As morbidades adquiridasna hospitalização de característica infecciosa contribuíram para complicações e mau prognóstico.Espera-se que outros estudos possam ser aprofundados no intuito de desenvolver protocolos deatendimento hospitalar ao idoso.

Objective: to estimate the incidence of red blood cell concentrate orders in elective femur fracture repair surgeries and to identify predictors for hemotranfusion. Method: retrospective cohort study conducted with 271 patients submitted to femur fracture repair surgery between July 2013 and July 2016. Surgical and transfusion data were obtained from patient charts and the Transfusion Management System. Association between sociodemographic and clinical variables related to the surgical procedure and the occurrence of red blood cell concentrate transfusion was analyzed using descriptive statistics, the chi-squared test, relative risk, and odds ratio. Multivariate analysis was performed using binomial logistic regression. Results: the incidence of blood orders for patients undergoing femur fracture repair surgery was 87%. Ninety-one (33.6%) patients received red blood cell concentrate transfusions. Even though placing blood orders is recommended, given the possibility of intra- or postoperative transfusions, only 52 (47.2%) blood transfusions occurred in the preoperative period. The variables female sex, low preoperative hemoglobin levels and procedure lasting longer than 120 minutes presented statistical significance (p<0.05) and were considered predictors for hemototransfusion. Conclusion: Perioperative nursing must be aware of the importance of blood orders for all patients undergoing femur fracture repair surgery, including in the preoperative period, with special attention to patients who are female, previously anemic and submitted to long-lasting procedures.
