A mass developed in the mandibular gingiva of a thoroughbred racehorse. When the horse could no longer eat unassisted, it was killed and immediately autopsied. Macroscopically, the mandible exhibited extensive osteolysis, with only a small amount of bone remaining around the tooth roots. The cut surface of the mass around the mandible consisted of neoplastic medullary tissue, in which osteogenesis was observed. The medullary tissue was composed of pleomorphic medium-sized to large cells, interlaced by collagen bundles. These cells had large, pale, round or ovoid, sometimes cleaved nuclei, with one or two prominent nucleoli. Mitoses were numerous. Electron microscopy showed that the cells in the medullary tissues were similar in shape to undifferentiated lymphocytes. Immunohistochemically, these cells were positive for B-cell associated antigen in the pre-B-cell stage. Our findings suggest that the horse had extranodal lymphoblastic lymphoma of suspected B-cell lineage, possibly originating from the lymphatic system of the gingiva. We consider that the osteolysis resulted from activation of osteoclasts by proliferation of the tumour cells.

Currently, quantitative studies of malignant lymphoma are being performed in an attempt to improve the classification of non-Hodgkin's lymphoma (NHL) for diagnostic, prognostic, and therapeutic purposes. Morphometric image analysis is one method that can be employed in cases of NIIL to obtain objective data of nuclear parameters; condensed chromatin being a compartment of the nucleus best measured at the ultrastructural level. This report assesses similarities or differences in the amount, distribution, and arrangement of condensed chromatin in nuclear profiles of normal and neoplastic lymphocytes in human surgical biopsy specimens. Morphometric data derived from electron micrographs of lymphocytes in germinal centers of lymph nodes with reactive hyperplasia (three cases) and small cell types of NHL (two examples of malignant lymphoma, well differentiated lymphocytic type (ML,WDL) and three cases of malignant lymphoma, poorly differentiated lymphocytic type (ML, PDL)) are compared. Results indicate that the distribution of condensed chromatin, i.e., the size of aggregates, and their spatial placement within the nucleus varies more than the amount 030th n e a n area per profile or mean volume) of this nuclear parameter, and that this applies to normal as well as neoplastic lymphocytes. When a series of condensed chromatin parameters were statistically compared, no major differences could be detected between lymphocytes in normal tissues and those in ML,WDL and ML,PDL, but considerable differences were found in each of the nuclear morphotypes in the individual cases within the groups. This degree of variation in nuclear characteristics within normal tissues and the two lymphoma categories has not been previously recognized. Clearly, the technique of morphometric analysis, as applied to electron micrographs, can provide new and useful data that must be appreciated if classification schemes currently used in NHL are to improve _ * and reflect biologic considerations.

Between 1968 and 1975, 44 evaluable children under 16 years of age with the histologic diagnosis of non-Hodgkin malignant lymphoma (ML) were treated at the Istituto Nazionale Tumori of Milan. Histologic diagnoses were reclassified as follows: 13 lymphoblastic (others) ML, 15 convoluted cell type lymphoblastic ML, 9 Burkitt type ML, and 7 immunoblastic ML. Only 36% of the patients had stage I and II disease. At diagnosis 25% showed malignant cells in the bone marrow smears. Bone marrow infiltration was particularly frequent in the convoluted cell type lymphoblastic ML and in the lymphoblastic (others) ML subgroups. Burkitt type ML frequently was associated with abdominal lesions and subsequently a high incidence of central nervous system involvement. Patients with stage I and II ML were encountered mostly in the immunoblastic ML subgroup. After 1973 more intensive chemotherapy plus radiotherapy seems to have slightly improved the survival of the patients, except in the Burkitt type ML Subgroup.
