Diabetic ophthalmopathy (DO) impairs patients' eyesight and even causes blindness. Here, we investigated the effect of 60% ethanol extract of the rhizome of Anemarrhenae asphodeloides (ERA), which is commonly used in Chinese medicine formulae in treating diabetes, on DO progression. Blood glucose, insulin, advanced glycation end products (AGE), super oxygen dehydrogenises (SOD), malondialdehyde (MDA) and glutathione peroxidase (GSH-Px) levels in serum and sorbitol concentration in the lens were measured. Retinal endothelium/pericyte (E/P) ratio was evaluated, and structural changes of the retina and lens were observed. Effects of mangiferin and neomangiferin, the two major components of ERA, on subnormal growth of pericytes induced by high glucose were also detected. It was found that the activities of SOD and GSH-Px in serum were increased, whereas MDA and AGE levels in serum and sorbitol concentration in the lens were decreased in ERA-treated DO rats. E/P ratio was decreased, and the pathological changes of the lens and retina were alleviated by ERA treatment. Moreover, the subnormal growth of pericytes induced by high glucose was ameliorated by mangiferin and neomangiferin. These results indicated that ERA could effectively prevent DO progression in streptozotocin-induced diabetic rats, and mangiferin and neomangiferin may be the main effective components.

Diabetic animal models studied to date have developed some lesions characteristic of the early stages of diabetic retinopathy. This spectrum of lesions includes degenerate and nonperfused (acellular) capillaries, loss of capillary cells, thickening of basement membranes, and in longer-lived species, microaneurysms and intra-retinal microvascular abnormalities. To date, none of these diabetic animal models has been found to reliably develop preretinal neovascularization (an advanced stage of the retinopathy), likely due in part to less vaso-obliteration occurring during the short duration of diabetes that these models have been studied compared to diabetic patients. Although not diabetic, some animal models develop a diabetic-like preretinal neovascularization, and these models have been used to study ways to inhibit the neovascularization. Animal models are being used to provide valuable insight into the roles of specific biochemical pathways or physiological abnormalities in the development of diabetic retinopathy. Distinct advantages and disadvantages of each of these models are outlined in this review, thus providing information that should be valuable for planning experimental studies pertaining to the retinopathy.

Intrasplenic implantation of syngeneic islets normalized the hyperglycaemia of alloxan-diabetic mice irrespective of whether the implantation was performed 2 weeks or 4 months after the induction of diabetes. Islet implantation soon after the onset of diabetes prevented the development of retinal capillary disease as evidenced by a normal ratio between endothelial cells and pericytes. The increase of the cell count ratio (endothelial cells/pericytes) induced by long-term diabetes could not, however, be restored by islet implantation. The present results further underline the importance of optimal treatment for prevention of diabetic retinopathy.
