Background: Cardiovascular disease is a kind of comorbidity of spondyloarthritis (SpA). Hyperuricaemia is a risk factor of cardiovascular disease. Previous SpA study concerning cardiovascular disease and hyperuricaemia found that hyperuricaemia is more prevalent in radiographic SpA. Thus, we evaluated the association of serum uric acid (SUA) concentration and radiographic SpA. Methods: We made use of data from a cross-sectional study in Chinese Shenzhen Second People’s Hospital from 2016 to 2018, which included 202 SpA patients diagnosed by rheumatologists. This study detected the prevalence of comorbidities and risk factors of SpA patients. Using data from this study, we evaluated the association between radiographic images of SpA and SUA concentration. We compared the SUA concentration between radiographic SpA group and non-radiographic SpA group, with logistic regression models. Stratified and interaction analyses were also performed to further confirm the consistence of the relationship. Multiple imputation was used to deal with missing data.Results: This research studied 202 SpA patients’ data including their SUA, sacroiliac images, and other relevant laboratory data. Elevated SUA concentration was found to be associated independently with the increased risk of radiographic SpA after adjusted for confounders. The risk for developing radiographic SpA increased 13% [adjusted odds ratio (OR) = 1·13, 95% confidence interval (CI):(1·05, 1·22), p=0·0014] for per 10umol/L SUA increased. Stratified analyses, interaction analyses and multiple imputation analyses also confirmed the consistence of the association.Conclusions: Based on this Chinese population cross-sectional study, we identify that elevated SUA concentration is a risk factor for developing radiographic SpA.

Purpose: We assessed the prevalence of metabolic syndrome (MetS) and associated factors in mainland Chinese patients with ankylosing spondylitis (AS). Patients and methods: A retrospective study was conducted in 117 AS patients and 117 age- and sex-matched healthy controls. Parameters of MetS based on the criteria established by the Chinese Diabetes Society in 2013 were tabulated. Demographic features, laboratory data, and clinical characteristics were also collected. Independent factors correlated with MetS in AS patients were identified by backward stepwise multivariate analysis. Results: The prevalence of MetS was higher in AS patients than in healthy controls ( P = 0.026). AS patients also had higher blood pressure and fasting serum glucose levels, but generally lower serum lipid levels. AS patients with and without MetS had no distinct differences in disease duration, medication usage, disease activity, or biomarkers of inflammation. Multivariable logistic regression analysis showed that hyperuricemia (odds ratio [OR] = 2.385, 95% confidence interval [95% CI] = 1.019–5.582, P = 0.045) and high body mass index (BMI, OR = 5.165; 95% CI = 1.935–13.787, P =0.001) were independent factors for MetS in AS patients. Conclusion: Chinese AS patients living in the mainland have an increased risk of developing MetS. Hyperuricemia and high BMI are predictors of MetS in AS patients.

Acute gout arthritis flares contribute dominantly to gout-specific impaired health-related quality of life, representing a progressively increasing public health problem. Flares can be complex and expensive to treat, partly due to the frequent comorbidities. Unmet needs in gout management are more pressing given the markedly increasing gout flare hospital admission rates. In addition, chronic gouty arthritis can cause joint damage and functional impairment. This review addresses new knowledge on the basis for the marked, inherent variability of responses to deposited urate crystals, including the unpredictable and self-limited aspects of many gout flares. Specific topics reviewed include how innate immunity and two-signal inflammasome activation intersect with diet, metabolism, nutritional biosensing, the microbiome, and the phagocyte cytoskeleton and cell fate. The paper discusses the roles of endogenous constitutive regulators of inflammation, including certain nutritional biosensors, and emerging genetic and epigenetic factors. Recent advances in the basis of variability in responses to urate crystals in gout provide information about inflammatory arthritis, and have identified potential new targets and strategies for anti-inflammatory prevention and treatment of gouty arthritis.
