“…Commentary: There are nine case series of treating rheumatoid vasculitis using TNF inhibitors; thus, the evidence level is V and the recommendation level is 1C. Serum TNF‐α levels are elevated in RA patients with rheumatoid vasculitis compared to those without rheumatoid vasculitis, and serum levels of CX3CL1 produced by vascular endothelial cells stimulated by TNF‐α are correlated with rheumatoid vasculitis severity . Therefore, TNF‐α may be closely involved in the pathogenic mechanism of rheumatoid vasculitis, while TNF inhibitors are expected to be sufficiently useful for treating rheumatoid vasculitis. Regarding the usefulness of TNF inhibitors for treating rheumatoid vasculitis, there are only case reports and one case series.…”

“…Mononeuritis multiplex is treated as a vasculitis with high-dose IV methylprednisolone and cyclophosphamide [72]. Whether anti-TNF agents are helpful remains unclear; although there are cases of successful treatment of refractory rheumatoid vasculitis [73,74], there is also increased incidence of anti-TNF agent-induced peripheral neuropathy [75]. As we have observed demyelinating neuropathies caused by these agents, we do not recommend them in cases of already existing neuropathy.…”
