Monkeypox – a description of the clinical progression of skin lesions: a case report from Colorado, USA

Abstract: A rapidly emerging global outbreak of monkeypox virus infection (MPXV) in over 50 non-endemic countries was identified in May 2022. We report the case and images of a patient with MPXV presenting with genital lesions later complicated by superimposed cellulitis in Colorado, USA. MPXV lesions are susceptible to bacterial superinfection, and with the advent of new cases, the early identification of skin lesions and their evolution during MPXV are imperative for treating clinicians. Clinicians should consider MPX… Show more

“…However, the current evidence is not de nitive since contact transmission is also a common route for MPV transmission. Moreover, lesions reported patients in the current outbreak are mostly detected in the anogenital region [16,24,25], which can signi cantly contribute to transmitting the infection since contacting such lesions during sexual activities is inevitable. Accordingly, future studies might be needed to explain further the pathogenesis of MPV presence in the seminal uid.…”

“…This pathogen causes a two-clinical stage illness, including a prodrome during its invasive period and a cutaneous phase defined as the skin eruption [ 7 ]. Although described over decades, the current ongoing outbreak outside Africa seems to present atypical clinical manifestations compared to cases reported before 2022 in endemic countries [ 8 10 ]. The MPXV, taxonomically, is currently part of the genus Orthopoxvirus, which belongs to the subfamily Chordopoxvirinae in the family Poxviridae.…”

“…(Anderson et al, 2003) Recently published literature has highlighted the need for clinical follow-up in MPX patients to allow early detection and treatment of bacterial superinfected lesions; however large patient series are missing to estimate this complication risk. (Ortiz-Martínez et al, 2022) The effect of HIV on the severity of Monkeypox is unknown. Although HIV positivity has not been associated with higher symptom severity in patients with good immunevirologic control, an exuberant cutaneous presentation has been described in an acute HIV setting.…”
