Monitoring of Aspirin (ASA) Pharmacodynamics with the Platelet Function Analyzer PFA-100®

Abstract: SummaryAnti-platelet drug therapy is currently performed without monitoring, because the established method of platelet aggregometry is cumbersome. The recently developed platelet function analyzer PFA-100® measures shear stress dependent, collagen epinephrine (CEPI) and collagen adenosine diphosphate (CADP) induced platelet plug formation. As the PFA-100 provides a valuable tool to detect patients with platelet dysfunction more efficiently and cost-effectively than aggregometry, we investigated its potential … Show more

“…At the individual level, increased CEPI-CT after aspirin intake correlates well with the basal CEPI-CT, indicating that subjects with low basal CEPI-CT may require higher aspirin doses to inhibit platelet plug formation (Homoncik et al, 2000). Our data suggest that inflammation-induced VWF release shortens CTs.…”

“…von Willebrand disease (VWD)]. It does so by measuring collagen-epinephrine (CEPI)-and collagen-ADP (CADP)-induced platelet plug formation under high shear stress conditions, and is strongly dependent on von Willebrand factor (VWF) levels in plasma (Fressinaud et al, 1999;Harrison et al, 1999;Homoncik et al, 2000).…”

“…Further investigations revealed a day-to-day variability of 9 to 12% and a diurnal variation as well, with about 30% longer CEPI-CT values in the afternoon than in the morning [30][31][32].…”

“…The time taken to occlude the hole is the closure time (CT), a measure of overall platelet-related haemostasis and this interval will be prolonged depending on the platelet activity. The use of two different cartridges with distinct agonists allows to distinguish the platelet function alterations due to intrinsic defects (principally by using CADP cartridge) or to antiplatelet therapy with ASA (CEPI cartridge) [93][94][95][96][97] , whereas the new Innovance cartridge is affected by thienopyridine therapy [98] . In comparison to BT test, this method is revealed more sensitive [27,46] especially for diagnosis of VWD and platelet function defects [25] .…”
