Monarthritis of the manubriosternal joint

Abstract: Six women and three males, who initially had seronegative monarticular arthritis of the manubriosternal joint (MSJ) were followed for a mean period of 7.1 years (range 0.8-17 years). Rather severe, progressive disease occurred in five patients, three of whom had pustulosis palmoplantaris (PPP) and one acne vulgaris. The course was milder in four patients who had either psoriasis vulgaris, psoriasis in the family or HLA-B27, and the disease remained localized to the MSJ in these patients. None of the patients d… Show more

“…The localized ACW sclerosis, present in seven females, was similar to the early changes in patients observed to develop more extensive sternal sclerosis [20,22]. It always occurred adjacent to uneven joint facets, but involvement of both joint facets, as seen in ordinary arthritis, only occurred at the manubriosternal joint.…”

“…It is possible that these patients had previously had sclerosis, which can disappear [22], but it is also possible that the PPP arthropathy may have different characteristics, being mainly localized either to bones or to joints. The finding of simultaneous occurrence of ACW symptoms and PPP only in patients without sclerosis who also had a higher age at the onset of PPP and ACW complaints (Table 2) may support this.…”

“…The course and appearance of radiographic ACW abnormalities were compared with those from a different series of 24 patients without generatised arthritis (series 2). Their ACW changes have previously been described [25][26][27], but additional follow-up information has been added. Nine of the 24 patients appeared with manubriosternal monarthritis initially.…”

“…Based on concomitant skin disease, 3 could be classified as having PPP, 2 psoriatic, 1 acne arthritis, and 1 reporting psoriasis in the family as having possible psoriatic arthritis. One patient had HLA-B27 associated changes, but the disorder of 1 patient was unclassifiable [25]. Eight of the 24 patients appeared with sclerosis and hyperostosis of the manubrium sterni, in 4 associated with PPP and in 1 with Ps plus PPP, whereas the disorder of 3 patients was unclassifiable [27].…”
