Molecular subgroups of medulloblastoma

Abstract: Recent efforts at stratifying medulloblastomas based on their molecular features have revolutionized our understanding of this morbidity. Collective efforts by multiple independent groups have subdivided medulloblastoma from a single disease into four distinct molecular subgroups characterized by disparate transcriptional signatures, mutational spectra, copy number profiles and, most importantly, clinical features. We present a summary of recent studies that have contributed to our understanding of the core me… Show more

“…Human medulloblastoma (MB) can be segregated into four major categories based on gene expression patterns: Hedgehog (HH) subtype, Wnt subtype, Group 3, and Group 4 (35,36). However, they all exhibit strikingly different gene expression profiles from AT/RT (37).…”

“…High resolution copy number arrays and transcriptional profiling platforms have been used by several groups of investigators to subclassify MB into at least four distinct subgroups with different demographics and prognoses (27,28). It has been proposed that stratification of MBs into one of these subgroups can offer better risk assessment to patients as well as potentially guide treatment decisions.…”

“…For example, a MB with monosomy 6 as the only abnormality is consistent with the WNT pathway subgroup, which has an overall good prognosis (27,28). In contrast, the presence of an i(17)(q10), with or without MYC amplification, places the tumor into either a poor (Group 3) or intermediate (Group 4) risk category (28).…”

“…Medulloblastoma is the most common malignant brain tumor in children that still has high cancer-related mortality and survivors often suffer from serious therapy-related side effects [2123]. Arising in the cerebellum, medulloblastoma is divided into four distinct subgroups: Wnt, Shh (Sonic hedgehog), and Groups 3 and 4 [22, 24]. Tumors of the Wnt group have a fairly good prognosis, but effective therapies for the other groups have yet to be identified.…”
