Molecular Evolution of the HIV-1 Thai Epidemic between the Time of RV144 Immunogen Selection to the Execution of the Vaccine Efficacy Trial

Abstract: The RV144 HIV-1 vaccine trial (Thailand, 2003 to 2009), using immunogens genetically matched to the regional epidemic, demonstrated the first evidence of efficacy for an HIV-1 vaccine. Here we studied the molecular evolution of the HIV-1 epidemic from the time of immunogen selection to the execution of the efficacy trial. We studied HIV-1 genetic diversity among 390 volunteers who were deferred from enrollment in RV144 due to preexisting HIV-1 infection using a multiregion hybridization assay, full-genome sequ… Show more

“…Approximately 90% of the incident infections in RV144 were infections with CRF01_AE, the predominant circulating strain in much of Southeast Asia (78,79). A sieve analysis identified two vaccine-associated genetic signatures in V2 corresponding to sites 169 and 181, further supporting the hypothesis that vaccinationinduced immune responses directed against the V2 loop were associated with protection (9).…”

“…In Thailand, during the early years of the epidemic, subtype B (Western type B and Thai B or B’) was the most prevalent clade among PWID while CRF01_AE was frequent in people infected heterosexually (Deng et al, 2008; Kijak et al, 2013; Ou et al, 1993). The ancestor of Thai B or B’ existed around 1985 having diverged from subtype B and became the founder strain in South and Southeast Asia (Deng et al, 2008; Junqueira and Almeida, 2016; Li et al, 2010b).…”

“…Participants in this cohort were identified during early acute HIV-1 infection and immediately offered cART; they were also willing to undergo colonic biopsies (26, 27). This cohort represents a unique opportunity to investigate the effect of cART on the GI tract at the earliest time points clinically feasible after initial HIV infection.…”
