Molecular determinants of vascular transport of dexamethasone in COVID-19 therapy

Abstract: Dexamethasone, a widely used corticosteroid, has recently been reported as the first drug to increase the survival chances of patients with severe COVID-19. Therapeutic agents, including dexamethasone, are mostly transported through the body by binding to serum albumin. Here, the first structure of serum albumin in complex with dexamethasone is reported. Dexamethasone binds to drug site 7, which is also the binding site for commonly used nonsteroidal anti-inflammatory drugs and testosterone, suggesting potenti… Show more

“…Life-threatening lung-damage and inflammation have been reported in critically and severely ill Covid patients. Dexamethasone and Prednisolone, two well-known exogenous glucocorticoids with their anti-inflammatory and immunomodulating properties may thus be able to ramp down the excessive COVID-19 symptoms (Favalli et al, 2020 ; Shabalin et al, 2020 ).…”

“…Furthermore, hyperthyroidism fosters albumin synthesis and secretion, and the contrary is observed in hypothyroidism [138]. Considering that albumin regulates dexamethasone distribution and bioavailability, thyroid dysfunction may affect dexamethasone pharmacokinetic influencing this variable [139].…”

“…Dexamethasone is transported in the body through binding to serum albumin. The binding site for dexamethasone in albumin is the same with testosterone and NSAIDs, and therefore this competition should be considered (Shabalin et al, 2020[ 116 ]). Dexamethasone is well tolerated as a widely used corticosteroid in short-term or single-dose use, but with increasing the dose or duration of therapy, a number of side effects appear.…”
