AIn recent years, infections due to multidrug-resistant gram-negative bacilli (MDR-GNB) are increasing worlwide. Carbapenem hydrolyzing enzymes (also known as carbapenemases) are responsible of carbapenem resistance which are mostly used in MDR-GNB infections. This resistance mechanism is different from other mechanisms of carbapenem resistance such as activation of multidrug efflux pumps in response to antibiotic exposure and impaired permeability due to porin mutations. However, all types of resistance mechanisms lead MDR-GNB infections MDR-GNB infections are associated with longer hospital stays and life-threatening severe infections. In this review, we aimed to present current data of carbapenemases.

Objectives:To reveal the relationship between clinical and environmental isolates, analyzing both phenotypic and molecular aspects, in an Acinetobacter baumannii (A. baumannii) epidemic, and to use the epidemiological data to determine the source of the epidemic, to identify potential risk factors, and inform the effort to prevent and manage future epidemics.Methods:Acinetobacter baumannii was isolated from 5 clinical samples in Sultan Abdulhamid Han Training and Research hospital, Istanbul, Turkey, for a week period. To determine potential sources of infection we established cultures surveillance. Microbiological identification and antibiotic susceptibility testing of A. baumannii were performed using conventional methods and automated identification system. Multiplex polymerase chain reaction (PCR) and pulsed-field gel electrophoresis (PFGE) were used for carbapenemase gene screening and clonal relationship evaluation.Results:Among the environmental samples, bacterial growth was observed in 3 of the sample cultures. Clinical and environmental samples collected from patients X and Y had phenotypically similar antibiotic susceptibility patterns. The clinical and environmental isolates from patients X and Y comprised the first cluster (6 isolates), the isolates from patient Z formed the second cluster (2 isolates).Conclusion:We detected that all outbreak-related isolates contained the same OXA-type carbapenemase genes. Phenotypic similarity, based on the analysis of antimicrobial susceptibility patterns, was correlated with genotypic similarity. These results suggest that monitoring antimicrobial resistance patterns with daily culture surveillance follow-ups, coupled with the use of amplification based methods to detect that clonal relationships are important for the early identification of outbreaks and rapid deployment of proper countermeasures to halt the spread of the causative agent.

ÖZAmaç: Non fermentatif gram negatif bakteriler ciddi nozokomiyal enfeksiyon etkeni olarak karşımıza çıkmaktadır. Bu çalışmanın amacı, kan kültürlerinden izole edilen non fermentatif Gram negatif bakterilerin dağılımlarının ve antibiyotik duyarlılık oranlarının incelenmesidir. Gereç veYöntem: Çalışmamızda, Ocak 2010-Aralık 2016 tarihleri arasında kan kültürlerinden izole edilen 134 Acinetobacter baumannii, 77 Pseudomonas aeruginosa, 9 Stenotrophomonas maltophilia ve 2 Burkholderia cepacia izolatlarının çeşitli antibiyotiklere duyarlılıkları retrospektif olarak değerlendirilmiştir. Saptanan bakterilerin tür tayini ve antibiyotik duyarlılık testi VITEK 2 (bioMérieux, Fransa) otomatize sistemi kullanılarak yapılmıştır. Test sonuçları 2010-2015 tarihleri arasında CLSI (Clinical Laboratory Standards Institute); 2016 tarihinde ise EUCAST (European Committee on Antimicrobial Susceptibility Testing) kriterlerine göre değerlendirilmiştir. Bulgular: İzole edilen suşlarda antibiyotiklere en yüksek duyarlılık oranları A. baumannii için kolistin (%97.0), P. aeruginosa için kolistin (%97.4), gentamisin (%90.9) ve seftazidim için (%88.3) saptanmıştır. A. baumannii ve P. aeruginosa suşlarında kolistin en etkili antibiyotik olarak saptanmıştır. Sonuç:Yüksek morbidite ve mortaliteye sahip bu enfeksiyonlarda bu tip çalışmaların her merkezde belirli aralıklarla yapılması ve kan kültürü laboratuvarlarının sonuçlarını hızlı bir şekilde ilgili kliniklere bildirmesi gerekmektedir. ABSTRACT QuantitativeObjective: Non-fermentative gram-negative bacteria are detected as agents of serious nosocomial infections. The aim of this study is to determine the distribution of nonfermentative gram-negative bacteria isolated from blood cultures and their antibiotic susceptibility rates. Material and Methods:In our study, susceptibilities to various antibiotics of 134 Acinetobacter baumannii, 77 Pseudomonas aeruginosa, 9 Stenotrophomonas maltophilia and 2 Burkholderia cepacia strains, which were isolated from blood cultures between January 2010 and December 2016, were retrospectively evaluated. Identification and antimicrobial susceptibilities of the detected bacteria were carried out using the VITEK 2 (bioMérieux, France) automated system. Test results between 2010 and 2015 were evaluated according to the CLSI (Clinical and Laboratory Standards Institute) criteria. However in 2016, they were evaluated according to EUCAST (European Committee on Antimicrobial Susceptibility Testing) criteria.Results: The highest susceptibility rates of A. baumannii strains isolated were determined for colistin (97%). The highest susceptibility rates of P. aeruginosa strains were determined for colistin (97.4%), gentamicin (90.9%) and ceftazidime (88.3%) Colistin was determined as the most effective antibiotic against A. baumannii and P. aeruginosa strains.
