Rationale: Mortality prediction is well studied in idiopathic pulmonary fibrosis (IPF), but little is known about predictors of premortality disease progression. Identification of patients at risk for disease progression would be useful for clinical decision-making and designing clinical trials.Objectives: To develop prediction models for disease progression in IPF.Methods: In a large clinical trial cohort of patients with IPF (n = 1,113), we comprehensively screened multivariate models of candidate baseline and past-change predictors for disease progression defined by 48-week worsening of FVC, dyspnea (University of California, San Diego Shortness of Breath Questionnaire [UCSD SOBQ]), 6-minute-walk distance (6MWD), and occurrence of respiratory hospitalization, or death. Progression outcomes were modeled as appropriate, by slope change using linear regression models and time to binary outcomes using Cox proportional hazards models. Measurements and Main Results:The overall cohort experienced considerable disease progression. Top-performing prediction models did not meaningfully predict most measures of disease progression. For example, prediction modeling explained less than or equal to 1% of the observed variation in 48-week slope change in FVC, UCSD SOBQ, and 6MWD. Models performed better for binary measures of time to disease progression but were still largely inaccurate (cross-validated C statistic <0.63 for >10% decline in FVC or death, <0.68 for >20-U increase in UCSD SOBQ or death, <0.70 for >100 m decline in 6MWD or death). Models for time to respiratory hospitalization or death (C statistic <0.77) or death alone (C statistic <0.81) demonstrated acceptable discriminative performance.Conclusions: Clinical prediction models poorly predicted physiologic and functional disease progression in IPF. This is in contrast to respiratory hospitalization and mortality prediction.

Интерстициальные заболевания легких (ИЗЛ) -ге терогенная группа заболеваний и патологических состояний, характеризующаяся различной степенью паренхиматозного неинфекционного воспаления (альвеолита и гранулематоза) и фиброза [1].Фиброзирующие альвеолиты (ФА) представляют собой группу ИЗЛ, различных по этиологии, осо бенностям патогенеза с преимущественным пораже нием интерстициальной ткани легких и развитием прогрессирующего легочного фиброза с исходом в «сотовое» легкое. К ним относятся идиопатичес кий ФА, экзогенные аллергический (ЭАА) и ток сический (ЭТА) альвеолиты и синдромы ФА при других заболеваниях. Пневмокониозы относятся к гранулематозам -ИЗЛ, характерной особен ностью которых является образование в легочной ткани гранулем в ответ на экзогенные (как правило профессиональные) воздействия.При воздействии внешних факторов неоргани ческой и органической природы (аллергенов, токси ческих факторов, фиброгенной пыли) нарушается структура мембраны пневмоцитов, активация син теза белков, которые рассматриваются как биомар керы ИЗЛ. Сывороточные биомаркеры наряду с компьютерной томографией и показателями внеш него дыхания могут быть использованы при диаг ностике, оценке активности, тяжести течения ИЗЛ, а также как прогностический фактор [2]. Биомарке ры могут применяться также для скринингового исследования при подозрении на профессиональ ные ИЗЛ. Среди исследуемых биомаркеров ИЗЛ особый интерес вызывают белки, синтезируемые пневмоцитами 2 го типа как клетками, непосред ственно участвующими в патогенезе этих заболева ний, в процессах повреждения и ремоделирования легочной ткани [3]. К таким белкам относятся преж де всего высокомолекулярный гликопротеин Krebs von den Lungen 6 (KL 6) и сурфактантные белки SP A и SP D. Значительно реже в качестве биомаркера ИЗЛ используется гликопротеин альвеоломуцин, также синтезируемый пневмоцитами 2 го типа.KL 6 является высокомолекулярным гликопро теином (200 кD), содержащим углеводородный ком понент MUC1. Он экспрессируется на апикальной поверхности пневмоцитов 2 го типа. При ИЗЛ про дукция KL 6 увеличивается, а имеющиеся деструк тивные изменения мембраны альвеолоцитов, обу словленные активацией протеаз, провоспалительных цитокинов, прежде всего фактора некроза опухо ли α и фактора роста фибробластов, приводят к по РезюмеУровень гликопротеина Krebs von den Lungen 6 (KL 6), альвеоломуцина и белка булавчатых клеток (club cells) -СС16 при экзогенных интер стициальных заболеваниях легких был исследован методом иммуноферментного анализа. Обследованы больные пневмокониозом (n = 13), экзогенным аллергическим (n = 26) и экзогенным токсическим (n = 20) альвеолитом в период активности и ремиссии заболевания. Уста новлено, что KL 6 и альвеоломуцин являются более информативными маркерами для оценки активности экзогенных фиброзирующих аль веолитов по сравнению с СС16. @ERSpublications Efficient IPF drug development is needed to ensure unmet medical need isn't overshadowed by scientific/financial risk http://ow.ly/K0aEvDespite the recent success of the nintedanib and pirfenidone programmes [1,2], idiopathic pulmonary fibrosis (IPF) remains a high-risk disease area for drug development. There are no adequate preclinical models in which to test potential compounds, and no established early clinical development pathway on which to make quick decisions regarding potential efficacy [3]. The historical approach in IPF of moving compounds directly from phase 1 safety and tolerability studies into large phase 2 studies with clinical end-points required major commitments from sponsors and research subjects. Now, with effective therapies reducing the frequency and responsiveness of clinical end-points, sponsors may decide that IPF is too costly a disease to invest in.IPF needs a more efficient and informative early clinical development pathway. The innovative study by WILKES et al. [4], published in this issue of the European Respiratory Journal, describing the phase 1 results of oral immunotherapy with type V collagen in patients with IPF illustrates one such approach. This study is worth careful consideration, both for the novelty of its potential therapy and for the approach it takes to three core study design issues. Issue 1: which mechanism to targetMost therapies under clinical development for the treatment of IPF target collagen production, the most salient (but also the most terminal) event in IPF pathobiology [5]. Less attention has been given to upstream events, and for good reason as the aetiology of IPF appears complex and heterogeneous, and knowing where to focus one's efforts has been a major challenge.
