Modulation of red blood cell population dynamics is a fundamental homeostatic response to disease

Abstract: Increased red blood cell (RBC) volume variation (RDW) has recently been shown to predict a wide range of mortality and morbidity: death due to cardiovascular disease, cancer, infection, renal disease, and more; complications in heart failure and coronary artery disease, advanced stage and worse prognosis in many cancers, poor outcomes in autoimmune disease, and many more. The mechanisms by which all of these diseases lead to increased RDW are unknown. Here we use a semi-mechanistic mathematical model of in viv… Show more

“…The increased RDW values in cardiovascular disorders is related to the triggering of the erythropoiesis by erythropoietin. On the other hand, elevated RDW values may be affected from RBC turnover (17,18). More recently, some authors mentioned that the higher values of the RDW is a bad prognostic factor in patients with cardiac diseases (19).…”

“…With an epidemiologic design, this study cannot determine biological mechanisms but can provide some hints to inform biological investigations searching for the mechanisms. Variations in RBC sizes measured by the numerator of RDW and by the clearance rate of RBCs were previously reported to be associated with prognosis (21,25). It was hypothesized that RDW elevation may result due to a greater %Micro (25), %Macro (23), or both (21).…”

It was hypothesized that RDW elevation may result due to a greater %Micro (25), %Macro (23), or both (21). This study confirms that it is all 3 (see Figure 5), showing that having concomitantly higher %Macro and higher %Micro is the highest risk condition across all morbidities and for mortality, with elevation of one or the other having an intermediate level of risk.…”

“…As well as mortality, in these patients increased RDW has been shown to predict advanced stage and worse prognosis (15). In a prospective, observational cohort study including 1,840 patients with solid and hematological cancer included in the prospective study named Vienna Cancer and Thrombosis Study (CATS), high RDW was found associated with an increased risk of mortality with an HR per 1% RDW increase of 1.11 (95% CI, 1.08-1.15, P<0.001).…”
