Obesity is a highly prevalent disease in the world associated with the disorders of endocrine system. Recently, it may be concluded that the only effective treatment of obesity remains bariatric surgery. The aim of the review was to compare the concepts of appetite hormonal regulation, reasons of obesity development and bariatric procedures published over the last decade. The reviewed publications had been chosen on the base on: 1. reasons and endocrine consequences of obesity; 2. development of surgery methods from the first bariatric to present and future less aggressive procedures; 3. impact of surgery on the endocrine status of patient. The most serious endocrine disturbances during obesity are dysfunctions of hypothalamic circuits responsible for appetite regulation, insulin resistance, changes in hormones activity and abnormal activity of adipocytes hormones. The currently recommended bariatric surgeries are Roux-en-Y gastric bypass, sleeve gastrectomy and adjustable gastric banding. Bariatric surgical procedures, particularly combination of restrictive and malabsorptive, decrease the body weight and eliminate several but not all components of metabolic syndrome. Conclusions: 1. Hunger and satiety are mediated by an interplay of nervous and endocrine signals. 2. Healthy adipose tissue secretion of adipokines is coordinated in an anti-inflammatory, insulin-sensitizing and cardioprotective pattern. However, with increasing fat mass this secretion pattern is changed into a proinflammatory, insulin resistant, atherogenic and fatal systemic environment. 3. Bariatric surgery is not a solution of the obesity problem for everyone. 4. Long term postsurgical observations of the hormonal profile changes are necessary and should be obligatory.

Background Gastric electrical stimulation (GES) has been applied to treat gastric motility disorders for decades. This study was designed to investigate the effects and mechanisms of GES for visceral hypersensitivity in a rodent model of functional dyspepsia (FD). Methods Male Sprague‐Dawley rat pups at 10‐days old received 0.1% iodoacetamide (IA) daily for 6 days. The experiments were performed when the rats reached 8‐11 weeks of age, and visceral hypersensitivity was established. Then, GES parameters were optimized and the chronic effects of GES on gastric hypersensitivity were assessed by electromyogram (EMG). Naloxone (3 mg/kg), D‐Phe‐Cys‐Tyr‐D‐Trp‐Orn‐Thr‐Pen‐Thr‐NH2 (CTOP, 1 mg/kg), and anti‐NGF (16 μg/kg) were individually intraperitoneally injected to investigate opioid and nerve growth factor (NGF) mechanisms. Tissues were analyzed for NGF expression. Key Results In the IA‐treated rats, the visceromotor response to gastric distension was significantly increased, and both acute GES with optimized stimulation parameters (0.25 seconds on, 0.25 seconds off, 100 Hz, 0.25 ms, 6 mA) and chronic GES (7 days, 2 hours/day) normalized gastric hypersensitivity. The inhibitory effect of GES on gastric hypersensitivity was blocked by naloxone and CTOP. Anti‐NGF normalized EMG responses in IA‐treated rats. The expressions of NGF in the tissues of IA‐treated rats were dramatically increased, and these increases were suppressed with GES. Conclusions and Inferences GES with optimized parameters improves gastric hypersensitivity induced by neonatal treatment of IA mediated peripherally by suppressing NGF and via the opioid mechanism involving the µ receptor. GES as a potential therapy for treating visceral pain may be explored in clinical studies.

Morphometric features of the fetal thyroid gland: ultrasound examination findings

The article discusses features of morphometric parameters of the thyroid gland based on results of ultrasound examination. The need to study development of the thyroid gland in the fetus is due to the high prevalence of thyroid pathology and the possibility of its early prenatal diagnostics.The aim of the study was to characterize morphometric parameters of the fetal thyroid gland at 20–24 weeks of gestation using ultrasound examination. Material and methods. In the study the thyroid glands of 70 fetuses, both male and female, at the age of 20-24 weeks, were exposed to ultrasound examination. During ultrasound examination, the authors evaluated linear parameters, an organ perimeter, thyroid volume, and growth rate. Results. Ultrasound examination of fetuses aged 20–24 weeks demonstrated that all anatomical structures of the thyroid gland were well visualized on the images. The period from the 20th to the 24th week of the fetal intrauterine development was accompanied by an increase in the studied parameters of the thyroid gland, the growth intensity ranged from 3% (width of the left lobe) to 37% (thyroid volume). In female fetuses, the lobe sizes of the thyroid gland and the index of the thyroid volume prevailed. This period was also characterized by a low (up to 14%) growth intensity of certain linear parameters in male and female fetuses in different age subgroups.Conclusion. Starting from the 20th week of intrauterine development, ultrasound examination allows detecting features of the topography, quantitative anatomy and growth intensity of the fetal thyroid gland. The data obtained evidence the possibility of assessing the state of the thyroid gland during ultrasound screening of a fetus.
