Background: Extremes of reproductive age are associated with adverse pregnancy outcomes. Although varieties of adverse outcomes are reported across international studies, it has been difficult to compare the results due to inconsistencies in design partly due to arbitrary age cut offs for low and high risk groups. This study utilized the international consensus age cut offs to separate low and high risk age groups and compared pregnancy outcomes among them.

Perinatal factors associated with developmental defects of enamel in primary teeth: a case-control study

The present study was designed to evaluate associations between developmental defects of enamel (DDE) in the primary dentition and aspects related to mothers and preschoolers in the city of Diamantina, Brazil. A case-control study was carried out involving children aged three to five years. The case group was composed of 104 children with at least one dental surface affected by DDE. The control group comprised 105 children without DDE, matched for gender and age. The diagnosis of enamel defects was performed using the Developmental Defects of Enamel Index. Information was collected through interviews investigating socio-demographic aspects, gestation, birth weight, prematurity and breastfeeding. Simple and multiple regression analyses were performed, providing unadjusted and adjusted prevalence ratios (Poisson regression). DDE were more prevalent among children who had not been breastfed (PR = 1.57; 95% CI: 1.1-2.2) and those whose mothers were under 24 years of age at the birth of the child (PR = 1.41; 95% CI: 1.1-1.9). The prevalence of DDE in the primary dentition was higher among children who had not been breastfed and those whose mothers were under 24 years of age at the birth of the child.

Influencia de la adolescencia y su entorno en la adherencia al control prenatal e impacto sobre la prematuridad, bajo peso al nacer y mortalidad neonatal RESUMENAntecedentes: La morbimortalidad materna y neonatal está incrementada en adolescentes y gestantes sin control prenatal o con una mala adherencia. Objetivo: Identificar la influencia de la adolescencia y su entorno sobre la adherencia al control prenatal y su impacto sobre la prematuridad y/o bajo peso al nacer y la mortalidad neonatal. Métodos: Estudio de casos y controles, realizado con 570 adolescentes y 2093 gestantes ≥20 años de edad, cuyos neonatos fueron hospitalizados en una unidad neonatal. Las asociaciones se determinaron mediante Odds Ratio, con su intervalo de confianza de 95%, empleándose la regresión logística en el análisis multivariado. Para la construcción del Modelo con Árboles de Clasificación y Regresión se emplearon variables que en el bivariado hubieran tenido un valor p<0,05. Resultados: La edad materna 13-19 años se halló asociada a mala adherencia al control prenatal, después de ajustarse por escolaridad <11 años, carencia de esposo o compañero permanente, carencia de seguridad social en salud y antecedente de gravidez ≥2 gestaciones. Esta mala adherencia al control prenatal se asoció con aumento de riesgo de prematuridad y/o bajo peso al nacer y de la mortalidad neonatal. Conclusiones: La adolescencia se halló independientemente asociada a una mala adherencia al control prenatal, sin embargo, existen otros determinantes importantes de esta mala adherencia, que a su vez se halló asociada con prematuridad y/o bajo peso al nacer, y con mortalidad neonatal.PALABRAS CLAVE: Adolescencia, control prenatal, prematuridad, bajo peso al nacer, mortalidad neonatal SUMMARYBackground: Maternal morbidity and neonatal mortality is increased in adolescents and pregnant women without prenatal care or poor adherence.
