The aim of this study was to compare postoperative drainage volumes and IMA levels in patients who underwent modified radical mastectomy (MRM) with using PlasmaBlade (PB) or electrocautery (EC). A total of 36 patients who underwent MRM with PB or EC in our clinic between August 2012 to February 2013 were enrolled. Number of removed and positive lymph nodes, duration of drainage and total drainage volume was recorded. Seroma formation after drain removal and number of aspirations were also recorded. Serum ischemia modified albümine (IMA) levels were analysed before surgery, 1 hour and 24 hour after surgery. In total, 36 patients were treated with MRM in the study period. Of the 36 patients, 16 underwent MRM with PB, and 20 underwent MRM with EC. The patients demographics were similar in both groups. The mean drainage volume and seroma formation were significantly higher in the PB group when compared with EC group (P<0.05). Number of aspirations due to the seroma were also high in PB group. The total aspiration volume of seroma was not different in both groups. IMA levels 24 hours after surgery in the PB group was significantly higher than EC group. There was no statistical significance between the groups for IMA levels at 1 st hour. PB is a monopolar energy device and is associated with increased levels of ischemia. This situation resulted with an increased volume of total axillary drainage and elevated risk of seroma formation.

Factors associated with greater debit drainage after axillary lymphadenectomy for breast cancer Introduction: Axillary lymph node dissection (ALND) is still a usual procedure in the treatment of breast cancer. A drain is normally placed in the surgical bed and maintained several days until obtaining a daily discharge of 30-50 ml, in order to reduce the appearance of seroma. The aim of this study was to analyze possible factors associated with an increased drainage volume. Patients and Methods: A retrospective study of all the patients undergoing ALND for breasts cancer at our institution between 2011 and 2012 was performed. Results: 40 females were included. There were no complications or mortality. Median hospital stay was 2 days. Median day of drainage removal was the 5th postoperative day. Mean total drainage volume was 298.9 ± 240.1 ml. High blood pressure associated with an increased drainage volume (550 ml vs 217.5 ml; p = 0.001) and a later removal (8 th vs 4 th day; p = 0.002). Similar happened with age > 65 years (420.8 ml vs 24.6 ml; p = 0.003) and (7th vs 5 th day; p = 0.009). The number of metastatic lymph nodes showed a direct correlation with total drainage volume (Pearson 0.503; p = 0.017) and removal day (Spearman 0.563; p = 0.006), similar to the primary tumour size: Total drainage volume (Pearson 0.447; p = 0.042) and removal day (Spearman 0.556; p = 0.009). Conclusion: Age over 65 years, high blood pressure, number of metastatic lymph nodes and primary tumour size associated with a higher drainage volume and time to remove the drain.

Background The LigaSure vessel sealing system allows dissection and hemostasis in a safe and quick way, and it has been reported to be an effective tool to shorten operative time and reduce intraoperative blood loss for various surgeries. However, short-and long-term outcomes in comparison with conventional surgery for gastric cancer resection are limited. Methods Between January 2005 and December 2009, 121 patients underwent curative resection for gastric cancer with LigaSure. Perioperative and long-term results were compared with those of 242 matched patients who received curative resection for gastric cancer with the conventional technique. Immediate operation outcomes, operation morbidity, recurrence and survival were compared between groups. Results In the LigaSure group compared with the conventional group, operation time was 156 versus 183 min (P \ 0.0001), intraoperative blood loss was 181 versus 236 ml (P = 0.042), intraoperative blood transfusion was 68 versus 161 ml (P = 0.014), and hospital stay was 11.9 versus 13.6 days (P = 0.001). There were no differences in operation morbidity, recurrence rates, overall and diseasefree survival between the LigaSure and conventional groups. Conclusions LigaSure is associated with shorter operation time and hospital stay, less blood loss and transfusion, and comparable operation morbidity and long-term outcomes in comparison with conventional surgery for curative gastric cancer resection.
