Modified anterior capsulotomy technique and histopathology of the anterior capsule in cataracts after prolonged exposure to intravitreal silicone oil

Abstract: Cataract development is common in phakic eyes after pars plana vitrectomy and exposure to silicone oil. Anterior capsule rigidity is frequently encountered during cataract surgery in these cases. We report a capsulotomy technique to overcome capsule rigidity, as well as the histopathological findings of the anterior capsule after prolonged exposure to silicone oil.

“…PPV surgery likely contributes to the progression of incipient or early cataract formation in those patients with cataracts present prior to surgery. Cataract formation in PPV patients is thought to occur via several mechanisms, including altered lens metabolism, inflammation, surgical trauma, and contact from SiO . A total of 12 patients (12 eyes) who had regained vision after successful PPV surgery underwent eventual phacoemulsification for removal of advanced cataracts.…”

“…However, in cases of a dense plaque, surgeons can either perform a posterior capsulorhexis or defer the management to a postoperative neodymium-doped yttrium aluminium garnet laser capsulotomy depending on their preferences and most importantly their judgment of the density of the plaque [38]. In cases with severe rigidity of the anterior capsule especially after long exposure to silicone oil, the surgeon can use a vitrector to create and complete the capsule opening or, alternatively, a microvitreoretinal blade and retinal microscissors can be used [39].…”

“…Anatomical changes that take place after pars plana vitrectomy prejudice the onset of complications. Yung et al [23] investigated the pathophysiological changes that occurred in eyes that underwent vitrectomy and admission of silicon oil. A hypertrophic response of the anterior capsule of about 20 -25 μm was observed.…”
