Emotions play an important role in decision-making and they can impact individual as well as shared decisions. With increasing complexity of the decision, the potential for emotions to influence the outcome increases. Emotions are thus an influential factor in oncological decision-making which is a complex and high-stakes situation. As the shared decisionmaking process is at the center of patient-centric decisions, we model emotions as social information that inform the shared decision-making process. We present and explain a range of emotional concepts, together with a specific clinical example, that can impact the shared decision-making process. Our process model shows that emotions are experienced in various combinations before, during, and after a shared decision is made and how patients' and physicians' emotions interact and spill over during a shared decision situation. Overall, our process model and specific example show how emotions can impact shared decision-making in oncology in a multitude of ways. With this paper, we want to raise awareness of the role of emotions in the shared decision-making process, as emotions are often not explicitly recognized as decision criteria. Increased awareness of emotions may help their optimal utilization and reduce their influence as a bias in shared decision-making.

Cancer, the deadliest disease in the world, is taking away the lives of millions of people. The disease and its property of metastasis are still understudied. Various therapies have been discovered to cure this malignancy, but nearly all of them introduce a lot of side effects. Therapies such as radiation, chemotherapy, surgery, etc., are in vogue but are not so economical and approachable for many needy people. Since the nature of cancerous cells is very complex among different individuals, it becomes even more complex to treat them. In modern times, biologically active compounds extracted from plants, weeds, and, most importantly, algae (marine drugs) found in the sea have proved to possess excellent anti-cancer potential. However, the major bottlenecks are the extraction of active substances in ample quantity with high quality. This chapter describes the role of microalgae as anticancer agents. Several aspects of bioactive compounds and challenges linked to microalgae will be discussed. A brief account of nanotechnology and its role in the treatment of cancer in the context of microalgae will be highlighted. The level of algal properties that affect cell proliferation, cell arrest, and apoptosis is elaborated. The current scenario of this investigation is extensively discussed in the study, along with the chemical structure, pros and cons.

Multidisciplinary tumor board analysis: validation study of a central tool in tumor centers

The established standard to ensure state-of-the-art cancer treatment is through multidisciplinary tumor boards (TBs), although resource- and time-intensive. In this validation study, the multiple myeloma (MM)-TB was reexamined, aiming to validate our previous (2012–2014) results, now using the TB data from March 2020 to February 2021. We assessed MM-TB protocols, physicians’ documentation, patient, disease, remission status, progression-free survival (PFS), and overall survival (OS) as left-truncated survival times. Moreover, TB-adherence, level of evidence according to grade criteria, time requirements, study inclusion rates, and referral satisfaction were determined. Within a 1-year period, 312 discussed patients were documented in 439 TB protocols. Patient and disease characteristics were typical for comprehensive cancer centers. The percentages of patients discussed at initial diagnosis (ID), with disease recurrence or in need of interdisciplinary advice, were 39%, 28%, and 33%, respectively. Reasons for the MM-TB presentation were therapeutic challenges in 80% or staging/ID-defining questions in 20%. The numbers of presentations were mostly one in 73%, two in 20%, and three or more in 7%. The TB adherence rate was 93%. Reasons for non-adherence were related to patients’ decisions or challenging inclusion criteria for clinical trials. Additionally, we demonstrate that with the initiation of TBs, that the number of interdisciplinarily discussed patients increased, that TB-questions involve advice on the best treatment, and that levels of compliance and evidence can be as high as ≥ 90%. Advantages of TBs are that they may also improve patients’, referrers’, and physicians’ satisfaction, inclusion into clinical trials, and advance interdisciplinary projects, thereby encouraging cancer specialists to engage in them.
