In this work, we present the first experimental upper limits on the presence of stochastic ultrahigh-frequency gravitational waves. We exclude gravitational waves in the frequency bands from (2.7−14)×10 14 Hz and (5−12)×10 18 Hz down to a characteristic amplitude of h min c ≈ 6×10 −26 and h min c ≈ 5 × 10 −28 at 95% confidence level, respectively. To obtain these results, we used data from existing facilities that have been constructed and operated with the aim of detecting WISPs (Weakly Interacting Slim Particles), pointing out that these facilities are also sensitive to gravitational waves by graviton to photon conversion in the presence of a magnetic field. The principle applies to all experiments of this kind, with prospects of constraining (or detecting), for example, gravitational waves from light primordial black hole evaporation in the early universe.

Национальный медицинский исследовательский центр кардиологии; 121552, Россия, Москва, 3-я Черепковская ул., д. 15А Резюме В обзоре рассматриваются основные положения новых Европейских рекомендаций 2019 г. по диагностике и лечению больных с хроническими коронарными синдромами (ХКС). ХКС определяются как различные эволюционные фазы ИБС, исключая ситуации при которых определяются и доминируют клинические признаки острого коронарного тромбоза (ОКС). Наиболее часто встречаются следующие клинические сценарии ХКС: 1) пациенты с подозрением на ИБC и «стабильными» синдромами стенокардии и/или одышкой; 2) пациенты с впервые появившимися признаками сердечной недостаточности или дисфункцией левого желудочка и подозрением на ИБС; 3) симптоматичные пациенты со стабильными симптомами менее 1 года после острого коронарного синдрома или пациенты с недавней реваскуляризацией миокарда; 4) асимптоматичные или симптоматичные пациенты со стабилизацией симптомов меньше 1 года после начального установления диагноза или реваскуляризации; 5) пациенты со стенокардией и подозрением на вазоспастическую стенокардию или микроциркулярную стенокардию; 6) асимптоматичные пациенты, у которых ИБС определяется при скрининге. Рассмотрены новые технологии, позволяющие улучшить диагностику и результаты лечения пациентов с ХКС, наиболее часто встречающимися у больных ИБС, определить клиническую вероятность обструктивного коронарного атеросклероза, высокий риск неблагоприятных событий для различных методов тестирования. Приводится ступенчатая стратегия длительной антиишемической лекарственной терапии больных ХКС со специфическими основными характеристиками (ЧСС, АД, функция левого желудочка). Показaно действие лекарственных препаратов как в плане уменьшения клинических проявлений, так и улучшения прогноза заболевания. Выделяют препараты первой линии (бета-блокаторы, антагонисты кальциевых каналов, короткодействующие нитраты) и второй линии (длительно действующие нитраты, ивабрадин, никорандил, ранолазин, триметазидин). Препараты второй линии назначаются пациентам с ХКС в случаях противопоказаний или плохой переносимости препаратов первой линии. Пациенты со стенокардией, обусловленной коронарной болезнью сердца, должны также принимать низкие дозы аспирина и статины. Рациональное сочетание медикаментозного и инвазивного лечения (чрескожное коронарное вмешательство или аортокоронарное шунтирование) у пациентов с ХКС позволяет значительно снизить риск сердечно-сосудистых осложнений и улучшить их качество жизни.

Ischemic heart disease (IHD) is one of the most common diseases of the cardiovascular system, which is based on a disorder of the trophism of the heart muscle against the background of pathological changes in the coronary vessels. The leading factor in changes in coronary vessels is stenosing atherosclerosis, which results in impaired blood flow. Risk factors include male gender, older age, a burdened familial history, as well as low physical activity, arterial hypertension, diabetes mellitus, and obesity. Ischemic heart disease is the leading cause of death, especially in developed countries, and the number one cause of medical visits related to heart and vascular problems. One of the most common manifestations of ischemic heart disease is angina pectoris. When pain sensations appear against the background of physical activity or emotional stress, it is called angina of effort, but if angina manifestations occur outside the period of physical and mental activity, we are speaking about resting angina pectoris. There are acute and chronic forms of ischemic heart disease, and while cardiologists and cardiac resuscitators usually deal with acute conditions, dynamic monitoring of patients after a myocardial infarction or with angina pectoris is a part of the responsibility of a general practitioner.
