BACKGROUND: Obesity is one of the most common children diseases. In present time it is being actively studied the natural hormones role in the pathogenesis of obesity. AIMS: To determine the relationship between serum nesfatin level with impaired lipid metabolism in school age children with overweight and obesity. MATERIALS AND METHODS: The study included children with overweight and obesity (case) and healthy children with normal body mass (control) aged 9 to 15 years. All children were determined by concentration in cholesterol blood and its factions, apoA-1, apoB. Enzyme-linked immunosorbent assay (Bio Human Nesfatin Enzyme Immunoassay Kit). RESULTS: Groups of patients with overweight and obesity (n = 53) and healthy children (n = 31) were comparable by age and sex. In obese children, a significant decrease in the level of nesfatin-1 was revealed compared with the group with normal body weight. In cases of children obesity there is a marked decrease in the concentration of nesfatin-1 in serum, especially expressed in the dyslipidemia presence. The nesfatin-1 level are negatively correlated to BMI and body fat content. CONCLUSIONS: The obtained data suggest that along with the known key role of nesfatin in the regulation of eating behavior and appetite, it also affects anthropometric indicators, primarily associated with the accumulation of excess fat mass. Based on the data obtained, nesfatin-1 should be considered as a possible marker for the development of obesity in children.

INTRODUCTION: The current stage of development of dental implantology permits to perform a complete dental rehabilitation even in complicated clinical situations. However, there are some difficulties which prevent achievement of a high success of implantological treatment in certain somatic pathologies that manifest in the form of systemic inflammation, and present relative contraindications for implantation. Important general somatic pathologies include chronic obstructive pulmonary disease, atherosclerosis, non-insulin-dependent diabetes mellitus, osteoporosis, etc., they are associated with the risk of acute inflammatory response of an organism to surgical implantological intervention. The risk of unpredictable complications also increases because of insufficiently wide laboratory diagnostic spectrum for planning dental implantation that includes a biochemical blood test, coagulogram, reaction to the presence of specific infections and viral diseases. This list of diagnostic data is not always sufficient for a complete prognosis of success of planned treatment in patients with a complicated general somatic or dental status. Besides, the interrelation between dental chronic diseases and general somatic pathologies which in some cases have common etiopathogenetic factors, is often not taken into account, but without the adequate diagnostic range they often remain unnoticed and lead to persistent complications in different periods after completion of treatment. Development of technologies of surgical and orthopedic dentistry permits to provide full dental care to patients even with different pathologies of organs and systems. But in this case, of importance is extended laboratory diagnostics aimed at identification of factors of chronic systemic inflammation, vascular risk, bacterial threats, for example, evaluation of some interleukins, tumor necrosis factor, C-reactive protein. CONCLUSION: A complex of clinical, laboratory and instrumental diagnostic methods will permit to adequately evaluate risk for treatment failure and construct a prognostic tactics of complex therapy, thereby reducing the probability for unpredictable complications in dental implantation in patients with different somatic pathologies.

Efficacy of whole-body cryotherapy in complex medical rehabilitation of patients after laparoscopic bariatric gastric bypass for the treatment of morbid obesity

The steady growth in the number of obese patients consequently leads to an increase in bariatric surgeries, while the issues of rehabilitation of these patients remain not fully resolved. Aim of the research: to evaluate the effectiveness of using the whole-body cryotherapy (WBC) in complex medical rehabilitation of patients after laparoscopic gastric bypass in the treatment of obesity. Materials and methods. An interventional, single-center, prospective, controlled, single-blinded, experimental, randomized study. The study included 2 stages. Stage 1: 40 patients underwent laparoscopic mini-gastric bypass with band-separated gastric pouch. Stage 2: the patients were divided into 2 groups: with the use of WBC as part of complex medical rehabilitation (Group A, n = 20) and without additional physiotherapy methods (Group B, n = 20). The course of WBC was 15 sessions of 1–3 minutes. The CryoSpace 2K device consists of a cryochamber (temperature –110 °С) and a pre-chamber (–60 °С). The procedures were carried out every other day; the exposure duration increased in parallel with the growth of cryoresistance. The patients were followed up for a total of 2 years. Results. Surgical results: the average duration of surgery was 85 ± 26 minutes; the length of stay in the hospital was 3.7 ± 1.5 days. One complication was registered. Initially, the patients in groups A and B did not differ in terms of sex, age, anthropometric data, and comorbidities composition. After 2 years of follow-up, BMI was significantly lower in group A with the use of WBC compared with group B (26.7 vs 29.4 kg/m2). The percentage of total weight loss, percentage of BMI loss and percentage of excess weight loss were significantly higher in group A. Conclusion. Taking into account the ongoing rehabilitation measures, the group with the use of WBC showed significantly better indicators for achieving ideal body weight and losing excess weight, which is an argument for recommending the use of this type of physiotherapy in the rehabilitation complex after bariatric procedures.
