The present meta-analysis was carried out to determine whether supplementation with glutamine (Gln) would reduce the intestinal inflammatory response and mucosal permeability in patients undergoing abdominal surgery. The PubMed, EMBASE, Web of Science, and The Cochrane Library databases were searched for randomized controlled trials on the effects of supplementation with Gln, and published from August, 1966 to June 2014. Inclusion criteria for the meta-analysis were: i) Study design was a randomized controlled trial, ii) study included patients undergoing abdominal surgery, iii) study patients received a supplementation with Gln peptide (Ala-Gln or Gly-Gln) whereas control patients did not use any supplements, and iv) study outcomes included inflammatory markers [C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), and interleukin (IL)-6, and IL-2 receptor] and markers of intestinal permeability [lactulose/mannitol, diamine oxidase, D(−)lactic acid, and endotoxin]. Qualities of controlled trials were assessed using the Jadad score. Meta-analyses were performed with fixed- or random-effect models depending on the heterogeneity of studies. There were 21 trials meeting the inclusion criteria. The meta-analysis revealed that the levels of CRP, TNF-α, and IL-6 in patients supplemented with Gln were significantly lower than those in control patients, whereas the levels of IL-2 receptor were increased by Gln supplementation. Gln also significantly decreased the lactulose/mannitol ratio, the levels of diamine oxidase and endotoxin, and tended to decrease the levels of cyclic D-lactic acid. In conclusion, Gln appears to effectively reduce the inflammatory response and intestinal mucosal permeability in patients after abdominal surgery.

A matrix removal procedure with ion-exchange resin prior to analysis for 18 fluorinated benzoic acids (FBAs) tracers in saline (>25% salt) reservoir water was optimized. The elimination of >98% of salt and the simultaneous matrix sample cleanup allowed the direct analysis using the supernatant by liquid chromatography coupled with tandem mass spectrometry (LC-MS/MS). This resulted in a gain in detection limits for most of the tracers in comparison with the reference method (direct analysis after minimum required dilution). The limits of detection (LODs) were in the range of 0.01–0.15 ng/ml and compared to other studies the developed method provided comparable limits of detection and advantage of simplified and shorter sample preparation. The presented method offers a considerable gain in simplicity and analysis time. Recoveries for all the tracers reached 80–100%, except for 2-FBA and 2,6-dFBA for which they were ca. 60%. The low recoveries were corrected by the use of five isotopically labeled internal standards. The method was validated by the analysis of spiked samples and by an independent comparison of the results with those obtained by solid-phase extraction LC-MS/MS method.Electronic supplementary materialThe online version of this article (doi:10.1007/s00216-016-0060-5) contains supplementary material, which is available to authorized users.

Aim: Glutamine administration influences intestinal permeability (IP). Enteral and parenteral glutamine supplementations have different metabolic pathways. In the present study, we investigated the effect of the route of glutamine supplementation on IP. The infection rate, inflammatory parameters and treatment outcome were the secondary end-points in this study. Patients and Methods: A prospective, single-blind study was performed in mechanically-ventilated, early enterally-fed, mixed-surgery, intensive care unit (ICU) patients, randomly assigned to the parenteral group (group P) or enteral group (group E). The supplemented groups were treated with glutamine for 5 days. IP was measured using the lactulose/mannitol (L/M) test at the end of the study. Results: A total of 81 patients completed the study; 39 from group P and 42 from group E. We found no difference in the L/M index (0.492 ± 0.68 group P, 0.521 ± 0.86 group E; P = 0.88) and the ICU-acquired infection rate (38 per cent group P, 28 per cent group E; P = 0.34). A positive correlation between the start of enteral feeding and the L/M index was confirmed (correlation coefficient: 0.36, P = 0.003), but not with the dose (correlation coefficient: −0.019, P = 0.9) of glutamine supplementation. The outcome data also showed no statistical significant difference. Conclusions:The results of this pilot study showed no difference in the IP, infection rate and 6 months' survival between the parenteral or enteral glutamine-supplemented patients.
