Modeling travel impedance to medical care for children with birth defects using Geographic Information Systems

Abstract: BACKGROUND Children with birth defects may face significant geographic barriers accessing medical care and specialized services. Using a Geographic Information Systems–based approach, one-way travel time and distance to access medical care for children born with spina bifida was estimated. METHODS Using 2007 road information from the Florida Department of Transportation, we built a topological network of Florida roads. Live-born Florida infants with spina bifida during 1998 to 2007 were identified by the Flo… Show more

“…Lack of health insurance is a crucial determinant of patients’ choosing to stay in their residence county for hospitalization (13). In addition to age, race/ethnicity, and health insurance status (1316), other factors that influence travel patterns are sex, severity of illness (13), patient location (15), and socioeconomic status (SES) (11,17). …”

“…Research has increasingly focused on geographic access to care among individuals with birth defects (Case et al, 2008; Fixler et al, 2012; Cassell et al, 2013; Delmelle et al, 2013; Radcliff et al, 2015). We characterized one-way drive time, one-way public transit time, and public transit availability to pediatric cardiac surgical care centers in New York for adolescents with CHDs.…”

“…The association between rural residence and increased travel time to appropriate care among those with birth defects, for example, has been well documented (Delmelle et al, 2013; Radcliff et al, 2015). In contrast to prior work, we found no association between defect severity and proximity to care (Cassell et al, 2013; Radcliff et al, 2015).…”

“…Although data from 108 Florida hospitals are represented in these data [11], not all hospitals are required to report to the AHCA. Most non-reporting hospitals do not provide newborn care [13].…”
