Mumps is an important childhood infectious disease caused by mumps virus (MuV). We reviewed the epidemiology, pathogenesis, and vaccine development of mumps. Previous studies were identified using the key words “mumps” and “epidemiology”, “pathogenesis” or “vaccine” in MEDLINE, PubMed, Embase, Web of Science, and Google Scholar. We excluded the articles that were not published in the English language, manuscripts without abstracts, and opinion articles from the review. The number of cases caused by MuV decreased steeply after the introduction of the mumps vaccine worldwide. In recent years, a global resurgence of mumps cases in developed countries and cases of aseptic meningitis caused by some mumps vaccine strains have renewed the importance of MuV infection worldwide. The performance of mumps vaccination has become an important issue for controlling mumps infections. Vaccine development and routine vaccination are still effective measures to globally reduce the incidence of mumps infections. During outbreaks, a third of MMR vaccine is recommended for groups of persons determined by public authorities.

GİRİŞ ve AMAÇ:Kızamık-Kızamıkçık-Kabakulak aşısında bulunan kızamık virüsü civciv embriyo hücrelerinde üretilmektedir ve yardımcı madde olarak jelatin içermektedir. Jelatin veya yumurta alerjisi olan kişilerde KKK aşısı uygulaması, sağlık personeli ve ailelerde kaygı oluşturmaktadır. Çalışmamızda, yumurta alerjisi olan hastalarda Kızamık-Kızamıkçık-Kabakulak aşılamasının güvenli olup olmadığının ve aşı öncesi test yapılmasının gerekliliğinin belirlenmesi planlandı. YÖNTEM ve GEREÇLER: Çalışma 2016-2018 yılları arasında prospektif olarak dizayn edildi. Kocaeli Üniversitesi Tıp Fakültesi Çocuk İmmünoloji ve Alerji polikliniğinde izlenen immunglobulin E (IgE) aracılı yumurta alerjisi tanılı hastalara, Kızamık-Kızamıkçık-Kabakulak aşısı öncesi ticari yumurta beyazı ve sarısı ile prik, taze (çiğ) yumurta ile prik, jelatin ve Kızamık-Kızamıkçık-Kabakulak aşısı ile prik, 1/100 sulandırılmış aşı ile intradermal deri testleri yapılması, yumurta spesifik IgE çalışılması planlandı BULGULAR: IgE aracılı yumurta alerjisi olup Kızamık-Kızamıkçık-Kabakulak aşısı yapılacak 62 hasta çalışmaya alındı. Hastaların median yaşı 12 ay (9-24 ay) olup %28'i (n= 17)'i kızdı. Hastaların 49 (%79)'u atopik dermatit, 8 (%12)'i ürtiker, 2 (%3,2)'si anafilaksi ve 2 (%3,2)'si atopik dermatit ve ürtiker birlikteliğiyle takip ediliyordu. Bir (%1,6) hastada klinik şikayet olmadan spesifik IgE ve cilt testi pozitifliği mevcuttu. Hastaların tümünde jelatin ile yapılan prik testler, aşı ile yapılan prik ve intradermal testler negatif saptandı. Tüm hastalara aşı tam doz uygulanarak hastalar 1 saat gözlem altında tutuldu. Yumurta ilişkili ürtikeri olan, ilk Kızamık-Kızamıkçık-Kabakulak aşısından sonra maküler döküntüsü olan bir hastada aşı uygulandıktan 30 dakika sonra aynı şekilde hafif maküler döküntü gelişti, tedavisiz geriledi. Diğer 61 hastada reaksiyon görülmedi. Reaksiyon oranı %1,6 (1/62) doz olarak bulundu. TARTIŞMA ve SONUÇ: Bulgularımız yumurta alerjisi olan hastalarda KKK aşısı öncesi test yapılmasının gerekli olmadığını ve aşının güvenle uygulanabileceğini düşündürmektedir. Life-Threatening Infections Due to Live-Attenuated Vaccines: Early Manifestations of Inborn Errors of Immunity

Live-attenuated vaccines (LAVs) can protect humans against 12 viral and three bacterial diseases. By definition, any clinical infection caused by a LAV that is sufficiently severe to require medical intervention attests to an inherited or acquired immunodeficiency that must be diagnosed or identified. Self-healing infections can also result from milder forms of immunodeficiency. We review here the inherited forms of immunodeficiency underlying severe infections of LAVs. Inborn errors of immunity (IEIs) underlying bacille Calmette-Guérin (BCG), oral poliovirus (OPV), vaccine measles virus (vMeV), and oral rotavirus vaccine (ORV) disease have been described from 1951, 1963, 1966, and 2009 onward, respectively. For each of these four LAVs, the underlying IEIs show immunological homogeneity despite genetic heterogeneity. Specifically, BCG disease is due to inborn errors of IFN-γ immunity, OPV disease to inborn errors of B cell immunity, vMeV disease to inborn errors of IFN-α/β and IFN-λ immunity, and ORV disease to adaptive immunity. Severe reactions to the other 11 LAVs have been described yet remain Bidiopathic,^in the absence of known underlying inherited or acquired immunodeficiencies, and are warranted to be the focus of research efforts. The study of IEIs underlying life-threatening LAV infections is clinically important for the affected patients and their families, as well as immunologically, for the study of the molecular and cellular basis of host defense against both attenuated and parental pathogens.
