Mixed Epithelial and Stromal Tumor of the Kidney: Preliminary Immunohistochemical and Electron Microscopic Studies of the Epithelial Component

Abstract: Mixed epithelial and stromal tumor of the kidney is a rare biphasic tumor composed of cysts and tubules embedded in the spindle cell stroma. Although the histogenesis of this tumor is unknown, it has been proposed that both components of the tumor, i.e., stromal and epithelial, are neoplastic. The authors report preliminary immunohistochemical and electron microscopic studies of the epithelial component from one case of a typical, benign, mixed epithelial, and stromal tumor of the kidney. In this study, some t… Show more

“…This is in agreement with the previous reports of malignant MEST, which also describe a benign epithelial component associated with malignant stroma [6][7][8]. Interestingly, a recent study by Picken and Fresco [12] suggests that the epithelial component of MEST may represent entrapped nonneoplastic tubules, which may explain our observation [13]. Malignantappearing cells were present throughout the stroma; curiously however, the regions of stromal condensation associated with the epithelial components, the ovarian-like regions, showed a greater degree of atypical cytologic features and more brisk mitotic activity relative to the regions more distant to the epithelial components where the stroma assumed a looser arrangement.…”

“…Its histogenesis remains uncertain. However, preliminary EM studies [32] suggest that the epithelial component may be entrapped rather than neoplastic. Again, additional EM studies may be helpful in addressing this issue.…”

“…Both entities show some remarkable similarities including sex predilection, age distribution, and morphological attributes of the epithelial and stromal components as well as immunohistochemical profile [15]. Albeit there are some variations in the individual categories with a higher prevalence of stromal-to-epithelial ratio, prominent ovarian stroma, smaller cysts more common in MEST; in contrast larger cysts with only thin septa (with septa being smaller than 5 mm) and in addition a lower stromal-to-epithelial ratio are typically observed in cystic nephroma [2].…”
