Mixed Endometrioid Adenocarcinoma and Müllerian Adenosarcoma of the Uterus and Ovary

Abstract: Mullerian adenosarcoma is a biphasic neoplasm composed of benign or atypical Müllerian epithelium and a malignant mesenchymal component that is usually, but not always, of low grade. Focal architectural or cytologic atypia of the epithelial component resembling atypical hyperplasia may uncommonly be present and foci of adenocarcinoma have been rarely reported. Whether the coexistence of these 2 tumor components is a result of independent primaries (collision tumor), adenocarcinoma arising from the epithelial c… Show more

“…Until recently, this unusual phenomenon has been described only in the form of single-case reports [ 6 , 7 ]. In 2021, El Hallani et al [ 5 ] published a series of 26 combined Mullerian AS and adenocarcinomas that included only cases of intimate admixture of both components. Twenty-two cases (84%) were located in the uterus, the rest in the ovary and pelvis.…”

“…The development of a composite/collisional neoplasm consisting of adenocarcinoma in AS is extremely rare. Until recently, only single case reports have been published, endometrioid carcinoma being an epithelial component of collisional tumor [ 5 ]. CCC as a collisional neoplasm in AS has never been described so far.…”

“…Adenosarcoma and endometrial adenocarcinoma are considered separate diseases, although endometrioid carcinoma mixed with clonally related adenosarcoma is reported [ 4 ]. Lenvatinib/pembrolizumab is active in metastatic or unresectable endometrial adenocarcinoma, despite low prevalence of PD-L1 expression [ 5 ] and regardless of MMR protein status [ 6 ].…”

“…OCS appear to follow a distinct natural history compared to other more common epithelial carcinomas. Their prognosis is dismal, and most patients relapse within one year after completion of initial treatment [33]. Within the samples tested here using TMAs, SORT1 expression was observed in about 60% of the benign tumor samples as well as in the germ cell and non-epithelial tumors.…”
