Introduction:In the early days of mitral valve replacement (MVR), there was an increased rate of mortality associated with low cardiac output syndrome (LCOS). Excision of the sub valvular apparatus (SVA) was one of the reasons for LCOS, as MVR at that time included complete excision of mitral leaflets and SVA that is, chordae tendinae and papillary muscles. Left ventricular function and geometry depend on dynamic interaction between left ventricular wall and mitral annulus which is bridged by SVA. Papillary muscle and chordae moderates wall tension during systole and optimize left ventricular distension during diastole. Interruption of ventricular-papillary-annular complex by excision of SVA during MVR thus causes impairment of normal ventricular stress-strain pattern and thus eventually leads to impaired left ventricular function and low cardiac output. In the early sixties Lillehei and his collegues capitalized on the fact and suggested that the high mortality rate associated with MVR could be reduced by preserving the papillary muscles and chorda tendenae. He preserved the posterior leaflet during MVR and noted a decreased incidence of post operative low output syndrome 1 . Short Term Clinical Outcome in Patients with Abstract:Background: Preservation of subvalvular apparatus (SAP) during mitral valve replacement (MVR) was introduced about forty years back, but the outcome of this procedure is not well studied yet. Our study aimed to measure the in-hospital outcome of this procedure in rheumatic patients.

Цель. Оценить ближайшие и отдаленные 5-летние результаты операции уни-версального хордосохраняющего протезирования митрального клапана. Материал и методы. в анализируемую группу вошли 103 пациента, которым за период с сентября 2009г по сентябрь 2014г в отделении дисфункции мио-карда и сердечной недостаточности ФГБНУ Российский научный центр хирур-гии имени академика Б. в. Петровского была выполнена операция универ-сального хордосохраняющего протезирования митрального клапана. возраст пациентов колебался от 21 до 75 лет и в среднем составил 55,2±0,9 года. Среди оперированных были 51 (49,5%) мужчина и 52 женщины (50,5%). Результаты. Из 103 пациентов, которым было выполнено универсальное хордосохраняющее протезирование, выписан 101. Отдаленные результаты были прослежены у 98 больных, что составляет 97% от всех выписанных. Сроки наблюдения составили от 3 месяцев до 5 лет. Функциональные резуль-таты операции более чем удовлетворительные: в I функциональном классе находится 66 пациентов (67,3%) от выписанных, во II -26 (26,5%), в III -3 (3,06%), умерло 3 (3,06%). У 89 пациентов (90,8%) отмечено увеличение функ-ционального класса сердечной недостаточности на 2 или даже на 3 ступени. в отдаленном периоде наблюдения в 25,5% случаях (25 больных) наблюдались пароксизмы фибрилляции предсердий. У 19 пациентов синусовый ритм был восстановлен медикаментозно и лишь у 6 (6,1%) мерцательная аритмия при-няла постоянную форму. У 15 пациентов, до операции имевших мерцательную аритмию, после операции восстановился синусовый ритм. Кровотечения различной локализации развились в 10 наблюдениях (10,2%) и были связаны с погрешностями в приеме антикоагулянтов непрямого действия и после кор-рекции уровня мНО не рецидивировали. Осложнения, так или иначе связан-ные с нарушениями терапии антикоагулянтами непрямого действия (тромбоз протеза, тромбоз левого предсердия, инфаркт миокарда, транзиторная ише-мическая атака головного мозга), зарегистрированы у 11 пациентов, что составляет 11,2% от всех наблюдений в отдаленном периоде. Заключение. Универсальное хордосохраняющее протезирование отвечает основным критериям, предъявляемым к технике протезирования митрального клапана: простота выполнения, сохранение физиологии левожелудочкового сокращения, безопасность для пациента и стабильность хороших результатов в отдаленном периоде наблюдения. данную методику можно рекомендовать к применению как при недостаточности митрального клапана, так и при его стенозе. Сохранение нормальной внутрисердечной анатомии левого желу-дочка позволяет улучшить его сократимость и повысить функциональный класс в состоянии пациента в отдаленном периоде после операции.
