Mitral valve replacement in children with rheumatic heart disease.

Abstract: SUMMARY We followed 110 children, ages 6-15 years, who had undergone mitral valve replacement for rheumatic heart disease. Group 1 consisted of 74 patients with mechanical prostheses and a mean follow-up of 6.3 years. Group 2 consisted of 13 patients with porcine xenografts followed for 3.3 years. Group 3 included 23 children with dura mater bioprostheses and a mean follow-up of 1.9 years. The clinical evolution of each group was assessed according to the New York Heart Association criteria. The incidence of c… Show more

“…Clinical improvement after surgery is evident. 23 The functional class before and six months after surgery for a group of 68 consecutive young survivors of all types of procedures for rheumatic valve disease is shown in Figure 2. Before surgery, 70.6% of patients were in functional classes III and IV.…”

“…Mitral valve disease, in particular, is present in all cases who have sequels of the rheumatic process. Replacement 9 "" or reconstruction 12 of the valve is often necessary in children under 16 years of age. In this young population, however, the differential diagnosis between congenital and rheumatic lesions is mandatory.…”

“…The incidence of reported complications has varied considerably. A high incidence of thromboembolic events occurring after valve replacement in children has been reported by Sade and colleagues [9], although other workers have reported a fairly low incidence of this complication, particularly those researchers using Starr valves [3,4,8]. In 1979 Sade and colleagues [9] reviewed the literature concerning these complications and recorded a 10% incidence of hemorrhagic problems following anticoagulant therapy.…”

“…During the next 3 years more descriptions of calcific valve failure appeared. These initially related only to the porcine prosthesis [3,6,12,[15][16][17][18][19][20], but more recently there have been a number of reports of calcific degeneration and valve failure occurring in bovine pericardial valves [21,22]. The change in pattern of valve usage seen in Fig.…”
