Mitral annuloplasty ring design and selection: Complete semi-rigid is best

Abstract: Mitral annuloplasty with prosthetic ring is an integral part of mitral valve repair. To understand the importance of this procedure we need to explore in depth the mitral annulus, its dynamics, and its important role in the mitral valve complex. This will lead us to conceptualize regarding design and best possible choice of valve ring in mitral valve repair.

“…These particular changes comprised wider rings with multi-layer fabric to allow for better stitching and more secure anchoring. Stiff or moderately flexible rings appeared to stabilize the annulus significantly better than highly flexible ones [ 10 , 11 ] so that the use of the latter did not play a role in this study. Procedural characteristics for the valvular apparatus changed markedly from mainly classical resection maneuvers to predominant use of artificial chordae [ 12 18 ].…”

“…There is increasing evidence of the benefits of the ring. Rigid or semi-rigid rings seem to stabilize the annulus considerably more effectively than highly flexible ones [ 11 , 12 ]; thus, the patients with flexible rings were not included in our study. In contrast, many different techniques are used to repair the leaflet and leaflet-retaining apparatus.…”

“…The Alfieri stitch, the surgical predecessor of the MitraClip, was one such technique. Nowadays, however, it is seldom performed surgically although being very fast [ 10 , 11 , 12 , 13 , 14 , 15 , 16 , 17 , 18 ]. During our 20 years of experience, we came across a simplification of the classic quadrangular resection maneuver by performing a reverse plication instead of resection or a neo-chordae implantation.…”

“…The surgeon's experience and preference are important in choosing the type of valve ring to ensure good long-term results. 16 , 17 , 18 Transesophageal echocardiography is used for assessment of mitral valve and repair results the same way as in other types of mitral valve. Both diastolic and systolic parameters as well as coaptation depth are measured for acceptability, prediction of success, and durability of the repair.…”
