Photorefractive keratectomy for correction of myopia was the first excimer laser-based technique to be developed. During the last 25 years, excimer lasers have improved technologically, several variations of the technique have been developed and pre-and postoperative pharmaceutical therapies have been investigated. This review article summarises these developments and the published meta-analyses on comparison of surface ablation techniques and laser in situ keratomileusis. The main conclusion is that there seem to be no differences between the clinical results obtained with the different variations of surface ablation techniques. When such a gas is put under electrical stimulation and high pressure, the excited compound can give up its excess energy by undergoing spontaneous emission, which very quickly (on the order of a picosecond) dissociates back into two unbound atoms. Directing the emitted light towards, for example, biological materials will result in ablation of surface molecules. In the early 1980s it was shown that using this approach biological materials, including the cornea, could be incised using the excimer laser.1-3 After short time, it was realised that actual surface ablation of the corneal stroma was possible after removal of the corneal epithelium 4 and that the resulting surface was very smooth. 5 In 1987, excimer laser-based keratotomy was performed in a human eye, 6 but it was the surface ablation application that pushed development of excimer lasers. 7,8 The first preliminary results in human eyes were published the late 1980s 9-12 and excimer lasers became commercially available around 1990. In the early 1990s larger follow-up series of patients undergoing PRK for myopia were published. [13][14][15] Since then, many modifications of PRK have been introduced, and a common uniting name for all these techniques is 'surface ablation'. G Ge er re eç ç v ve e Y Yö ön nt te em ml le er r: : Viral keratokonjonktivit sekeli yüzeyel kornea opasitelerinin tedavisinde PTK uygulanmış 21 hastanın 32 gözü geriye dönük değerlendirildi. Tüm hastaların görme keskinlikleri, refraktif durumları ve korneal yüksek sıralı aberasyonları PTK öncesinde ve sonrasında değerlendirildi. PTK öncesi ve sonrasındaki 12 ve 24. aylarda tüm hastalara ön segment optik koherens tomografi çekildi ve korneal topografik inceleme yapıldı. Korneal yüksek sıralı aberasyonlar 3 mm'lik santral zonda kombine Scheimpflug-Placido disk topografi cihazı ile değerlen-dirildi. B Bu ul lg gu ul la ar r: : Hastaların ortalama takip süresi 26,4±4,0 (23-32) ay ve ortalama yaşı 24,2±8,4 yıl idi. Görme keskinliğindeki bozulmanın süresi ortalama 13,2±4,2 (12-18) ay idi. Düzeltilmiş ve dü-zeltilmemiş görme keskinliklerinde PTK sonrası 12 ve 24. aylarda anlamlı artış saptandı. İşlem sonrası hipermetropik kayma 12. ayda 0,62±0,28 dioptri iken, 24. ayda 0,58±0,35 dioptri bulundu. Toplam korneal yüksek sıralı, koma, trefoil ve sferik aberasyonlarda anlamlı azalma saptandı. Visual acuity, manifest refraction and high order aberrations were identified for all patients preoperatively and postoperatively. Corneal topography and anterior segment optical coherence tomography were also evaluated preoperatively and at 12 and 24 months postoperatively. High order corneal aberrations were assessed with the use of a Scheimpflug camera combined Placido disc topography in the central 3-mm corneal area. R Re es su ul lt ts s: : The mean follow-up period was 26.4±4.0 (23-32) months and the mean age was 24.2±8.4 years. The patients were suffering from the impaired vision related to corneal opacities for 13.2±4.2 (12-18) months.
