Mitigating HPA axis dysregulation associated with placement changes in foster care

Abstract: Summary Maltreated foster children often exhibit alterations in diurnal hypothalamic-pituitary-adrenal (HPA) axis activity that are characterized by lower cortisol levels upon waking and smaller declines in morning-to-evening cortisol levels. Previous research has shown that this dysregulated pattern is associated with high caregiver stress levels over the course of foster care placements. In contrast, therapeutic interventions that emphasize consistent and responsive caregiving have been associated with more … Show more

“…Although physiological reactivity in humans can be altered in the short term by psychosocial interventions (34,35), including in children exposed to maternal deprivation (36,37), we are unaware of experimental research examining whether random assignment to a caregiving environment alters patterns of physiological reactivity later in development. Finally, although a sensitive period exists during which the environment has particularly strong influences on stress response system development in animals, studies that can rigorously identify such a period in humans are lacking.…”

“…15,17 Another study found that placement changes predicted dysregulation in cortisol for the control group only. 18 Our study adds to the growing body of literature by examining the impact of a child-focused intervention on permanency and placement outcomes. The Fostering Healthy Futures (FHF) program is a 9-month preventive intervention for preadolescent children aged 9 to 11 who were recently placed in foster care because of child maltreatment.…”
