Missed anatomy: frequency and clinical impact

Abstract: It is generally accepted that a major cause of the failure of root canal therapy is an inability to localize and treat all of the canals of the root canal system. The risk of missing anatomy during root canal treatment is high because of the complexity of the root canal system. All categories of teeth may have extra roots and/or canals, but the likelihood of finding aberrant canal configurations is higher in premolars and molars. In addition, lateral ramifications of the root canal system may be present in all… Show more

“…The incidence of apical periodontitis is directly proportional to complexity of the root canal system. Based on few epidemiological and retrospective studies Cantatore et al 8 had discussed that mishap of missed canal resulted in high percentage of apical periodontitis of failed endodontically treated teeth. The prevalence of post treatment disease was more inclined towards the multirooted molars where the possibility of missing an extra canal is more than the incisors.…”

“…Anatomic variations within the root canal system are a challenge for the dental practitioner. Extra roots, missed canals, variable morphology, ramifications, isthmus etc result in an incomplete cleaning and disinfection of the pulp space [7]. This may lead to treatment failures.…”
