Misclassification of Men with Reported HIV Infection in Ukraine

Abstract: We analyzed data on reported mode of transmission in case reports of HIV-infections among men in Ukraine. The number of men who were reported to have acquired HIV through heterosexual transmission increased substantially in 2006-2011. However, we estimate that up to 40 % of reported cases of heterosexual transmission among men may actually represent misclassified men who have sex with men or persons who inject drugs. These findings indicate a need to improve the quality of data on reported mode of HIV transmis… Show more

“…Our experience over the past 15 years working in Ukraine is that most street users will not go to AIDS Centers or military recruitment facilities (for example), to be tested, as there is little benefit to be gained and much to lose (e.g., stigma, discrimination, etc.). Moreover, a recent study by J-I Cakalo, et al found that misclassification of PWID occurred frequently [43]. The authors estimated that as many as 34.5% of men reported as heterosexual from 2005 to 2011 could have actually been PWID.…”

“…Though previous surveillance suggested a generalizing epidemic in Ukraine (4), new information triangulating multiple data sources confirms that HIV remains concentrated in key populations (5). Among PWID, who account for the majority of cumulative HIV cases, HIV prevalence exceeds 40% (6, 7).…”

“…59 Required name-based HIV registries often undermine voluntary testing efforts and treatment engagement. 98,108 Officially reported HIV data therefore underestimate true prevalence, 118 with restriction of access to HIV treatment due to mandatory registration combined with stigma, discrimination, and criminalisation of key populations. 6,110,119 Similarly, patients receiving opioid agonist therapy must be officially registered before receiving it in all EECA countries, which can lead to restrictions on employment opportunities, limitations in housing, and revocation of drivers’ licences, further compounding economic disparities.…”
