Minocycline-Associated Tooth Staining

Abstract: OBJECTIVE: To describe a case of tooth discoloration in an adult after minocycline treatment for arthritis. CASE SUMMARY: A 68-year-old white woman presented with blue–black staining of her lower anterior teeth after 4 months of minocycline therapy for arthritis. “…25 Staining of the teeth is thought to be due to the formation of a tetracycline-calcium orthophosphate complex during tooth development. 26 The staining tends to worsen over time due to an ongoing photochemical reaction involving the tetracycline crystals embedded in the crystalline structure of the teeth. 25 Our patient' s adverse effect was not permanent; manual descaling of her teeth removed the discoloration entirely, with no recurrence.…”

“…However, the disadvantages of this mixture are not well-known. Among the components of the mixture, minocycline, a derivative of tetracycline, can induce tooth discoloration after long-term oral use (10)(11)(12)(13). Kim et al (14) suggested that Ledermix (Haupt Pharma GmbH, Wolfratshausen, Germany) paste introduced in the canal might cause discoloration of teeth, and that such effects might be due to the demeclocycline in the Ledermix paste.…”

“…Apart from black coloration of the thyroid, pigment changes of the skin, nails, teeth, blood vessels, heart valves, and bones have also been reported in patients chronically taking minocycline [3,5,8,12]. In addition, black discoloration phenomena of other organs in the course of natural diseases, such as "black esophagus" [14], may occasionally be observed at autopsy.…”
