Minimally invasive multiple lymphaticovenular anastomosis at the ankle for the prevention of lower leg lymphedema

Abstract: This minimally invasive preventive LVA procedure might prevent lymphedema and improve the physical appearance of the limb with minimal scarring. Long-term follow-up will be necessary to monitor the future progression of edema in these patients.

“…23 Preventive lymphaticovenular anastomosis (LVA) was proposed as a preventive procedure in the contralateral limb of patients with secondary unilateral lower limb lymphedema due to the possibility of future lymphedema in the contralateral leg. 24 The concept of LYMPHA arises from broad clinical experience with performance of MLVA for the treatment of chronic peripheral lymphedema. 25,26 Compared with pedicled omentoplasty, LYMPHA is an easier technique to perform and does not require laparotomy or laparoscopy.…”

“…Most studies reporting the use of this technique for LE prevention have been performed in heterotopic locations, requiring longer operative times. 29,42,43 The additional operative time for studies performing prophylactic telescopic LVAs ranged from 15 to 45 minutes while for heterotopic supermicrosurgical LVAs varied between 26 to 120 minutes. 15 Surgical prevention of LE demands a combination of minimizing nodal excision, accurate identification and preservation of specific lymphatic pathways, and reconstruction of disrupted lymphatics that are critical.…”

“…The array of surgical interventions for prophylaxis of LEL is more diverse compared with UEL and varies also on the lymphadenectomy site since prevention of LEL may be considered after paraaortic, ilio-inguinal, or ILND; ►Table 2). 29,39,40,42,43,52,53…”

“…En el caso de linfedema de extremidad inferior secundario a la linfadenectomía pelviana por tratamiento oncológico, la prevención del linfedema de la extremidad contralateral se puede lograr realizando múltiples anastomosis linfático-venosas (LVA) 15 o anastomosis linfático-venosas de los vasos eferentes de linfonodos inguinales 16 antes que aparezcan los síntomas. Esto se indica cuando existe disfunción linfática diagnosticada con linfografía ICG en la extremidad sana (linfedema subclínico).…”
