Minimally invasive direct repair of lumbar spondylolysis with a pedicle screw and hook construct

Abstract: Object Lumbar spondylolysis occurs in approximately 6% of the population and presents with localized mechanical back pain, often in young athletes. Surgical treatment may involve decompression, lumbar intersegmental fusion, or direct repair of pars defects. Although such open procedures may effectively resolve symptoms, minimal-access approaches may additionally decrease collateral damage to soft tissues, allowing young, active patients to resume athletic activities … Show more

“…Eight studies used pedicle screws with laminar hooks, and 4 studies used pedicle screws with a wiring technique. The proportion of patients with low-grade spondylolisthesis ranged from 5.26% 33 to 100% 7,30,47 in the included studies. While most studies included cases of low-grade spondylolisthesis, the exact proportion of cases was mentioned in only 16 studies.…”

“…Analysis of this group showed excellent fusion rates; fusion rates of 100% were noted in several studies. 20,21,25,30,37,47,50 The pooled fusion rate was 90.21%. The pedicle screw group had the lowest pooled complication rate of 12.8%.…”

“…6 Other authors have reported using pedicle screws to secure the lamina with either a rod-hook construct or a V-shaped rod under the spinous process. 1,12,29,39 Basic Surgical Technique. A standard midline approach to the lumbar spine is performed with care taken to preserve the multifidi attachment to the lateral capsules of the L4-5 and L5-S1 facet joints (unless pedicle screws will be used) and to keep the supraspinous and interspinous ligaments intact.…”

“…Direct repair of the injury without arthrodesis and with simple blood irrigation of the focus and external immobilization, or compression with hooks and screws, is the preferred treatment, but cases with spondylolisthesis and significant instability make segmental arthrodesis necessary. 11,[34][35][36] The safe and correct interpretation of the imaging exams is fundamental to a decision on the best type of treatment.…”
