Minimally invasive and remote‐access thyroid surgery in the era of the 2015 American Thyroid Association guidelines

Abstract: Thyroid surgery has evolved throughout the years from being one of the most dangerous surgeries to becoming one of the safest surgical procedures performed today. Recent technologic innovations have allowed surgeons to remove the thyroid gland from a remote site while avoiding visible neck scars. There are many endoscopic approaches for thyroidectomy. The most common cervical approach is the minimally invasive video‐assisted technique developed by Miccoli et al. The robotic transaxillary and axillary breast ap… Show more

“…The main advantages of this technique are that the scars are completely invisible as they are situated intraorally, and midline access ensures that there is excellent access to bilateral thyroid lobes and central compartment lymph nodes . In addition, the use of wristed instruments, elimination of hand tremor, use of additional arms, and the superior visualization of the da Vinci camera gives TORT further precision during thyroid surgery …”

“…18 These innovations have taken various endoscopic and robotic forms, including axillary, facelift, breast, and transoral methods. 19 Of the various transoral approaches to the thyroid, the transoral vestibular approach is the most cited in the literature and has been shown to have good surgical outcomes with the least complications in Western populations. 20 First described by Richmon and colleagues, 21 the transoral vestibular approach offers several technical advantages over other remote-access neck surgeries.…”

“…These techniques include endoscopic or robotic breast, bilateral axillobreast, and axillary incisions, as well as the retroauricular approach . More recently, transoral thyroidectomy has become a favored approach as it provides midline access to the thyroid with minimal extracervical tissue dissection and no cutaneous scar …”

“…5 More recently, transoral thyroidectomy has become a favored approach as it provides midline access to the thyroid with minimal extracervical tissue dissection and no cutaneous scar. 2,[6][7][8][9][10][11][12] Despite this experience, the North American experience has been decidedly different. 5 Early excitement for remote access thyroidectomy was tempered when novel complications were reported via the transaxillary approach.…”
