Minimalist approach for transcatheter aortic valve replacement

Abstract: Transcatheter aortic valve replacement became a routine interventional procedure worldwide, together with improvements in valve technology and optimized implant techniques. The concept of minimalist approach became relevant, aiming at optimal technical implant, fewer perioperative comorbidities, and consequently faster recovery and less associated risks. Compared with surgical aortic valve replacement, reduction of hospital and intensive care unit stay and fewer rates of procedural complications offset the hig… Show more

“…As a result, the procedure is performed using only local anesthesia and conscious sedation. 2 MA-TAVR has been shown to decrease procedure times, shorten hospital length-of-stay, and yield quicker patient recovery without compromising postprocedural results. [3][4][5][6][7][8][9] While there has been substantial progress in therapeutic delivery devices for complex SHI, the integration of information from independent x-ray and echo image displays remains challenging for the interventionalist.…”

“…As operator confidence has increased and procedural complications have declined, there has been a gradual shift towards “minimalist approach” TAVR (MA‐TAVR) in which intraprocedural TEE is replaced with postprocedural TTE immediately after TAVR valve deployment. As a result, the procedure is performed using only local anesthesia and conscious sedation 2 . MA‐TAVR has been shown to decrease procedure times, shorten hospital length‐of‐stay, and yield quicker patient recovery without compromising postprocedural results 3–9 …”

“…8,9 Therefore, IVL provided adequate approach to perform TAVI in a fast, safe and efficient manner, with no need for surgery to obtain alternative accesses, and enable treatment of the patient as per the minimalist protocol, with early discharge in less than 24 hours after the procedure, which might not be possible in case of an alternative surgical access. 10 Hence, IVL is a new important tool in the scenario of challenging peripheral vascular accesses.…”
