IntroductionA introdução da técnica laparoscópica em 1985 foi um fator importante na colecistectomia por representar técnica menos invasiva, resultado estético melhor e menor risco cirúrgico comparado ao procedimento laparotômico.AimTo compare laparoscopic and minilaparotomy cholecystectomy in the treatment of cholelithiasis.MethodsA systematic review of randomized clinical trials, which included studies from four databases (Medline, Embase, Cochrane and Lilacs) was performed. The keywords used were "Cholecystectomy", "Cholecystectomy, Laparoscopic" and "Laparotomy". The methodological quality of primary studies was assessed by the Grade system.ResultsTen randomized controlled trials were included, totaling 2043 patients, 1020 in Laparoscopy group and 1023 in Minilaparotomy group. Laparoscopic cholecystectomy dispensed shorter length of hospital stay (p<0.00001) and return to work activities (p<0.00001) compared to minilaparotomy, and the minilaparotomy shorter operative time (p<0.00001) compared to laparoscopy. Laparoscopy decrease the risk of postoperative pain (NNT=7) and infectious complications (NNT=50). There was no statistical difference between the two groups regarding conversion (p=0,06) and surgical reinterventions (p=0,27), gall bladder's perforation (p=0,98), incidence of common bile duct injury (p=1.00), surgical site infection (p=0,52) and paralytic ileus (p=0,22).ConclusionIn cholelithiasis, laparoscopic cholecystectomy is associated with a lower incidence of postoperative pain and infectious complications, as well as shorter length of hospital stay and time to return to work activities compared to minilaparotomy cholecystectomy.

Background:: Reports of open-Cholecystectomy (OC) or laparoscopic Cholecystectomy (LC) under general anesthesia (GA) in the surgical treatment of gallbladder disease are common, but those performed under local anesthesia (LA) are much more limited, especially for old aged patients unfit for GA. Objectives: This study try to show that Mini-Cholecystectomy (MC) under LA for symptomatic gallstone disease is safe and effective in old aged patients, unfit for GA. This study tried to determine the safety and success of Mini-Cholecystectomy under local anesthesia for symptomatic gallstone disease in old aged patients unfit for GA. Methodology: ninety patients with gallstone disease scheduled for Mini-Cholecystectomy under local anesthesia were included in this prospective study in Erbil teaching hospital and sardam private hospital. Seventy-three patients were female, with a median age of 70 years (range, 50-90). All of the patients had evidence of acute cholecystitis. Mini-Cholecystectomy MC was performed by a standardized technique and under the combination of local anesthesia (1% zylocain without adrenaline) and intravenous administrations of fentanyl (0.001-0.002 mg/kg) and midazolam (0.05-0.1 mg/kg). Statistical analyses were performed using Microsoft Word Excel 2007. Results: The median operative time was 45 minutes (range, 35-70). Local anesthesia was converted to general anaesthesia in seven patients owing to the severe discomfort caused by lysis of dense adhesions around the gallbladder, giving a success rate of 95.5%. Cholecystectomy was done successfully in 65(72.22%) patients, while cholecystostomy was performed in the remaining 25 (27.77%) patients because of the severe adhesions that rendered Cholecystectomy very difficult. The median hospital stay was 2.6 days (range, 2-7). Conclusions: Mini-Cholecystectomy (MC) under Local anesthesia (LA) is an effective surgical procedure for old aged patients with acute calculus cholecystitis who are unfit for GA. Recommendation: It is recommended to study a larger sample for long period to evaluate the late complications of the operation.

Thoracic spinal anaesthesia for abdominal surgery in a humanitarian military field hospital: a prospective observational study

BackgroundProviding safe and effective anaesthesia in the context of humanitarian medicine is always a challenging situation. Spinal anaesthesia, and in particular its thoracic approach, represents a promising technique for such a limited-resource environment. This prospective observational study investigated the feasibility of thoracic spinal anaesthesia (TSA) for abdominal surgery in a field-deployed military hospital.Materials and methodsWe included adults scheduled for elective open cholecystectomy in a field hospital. Patients received TSA at the T9–T10 level. The primary outcomes were the feasibility of surgery under TSA and the haemodynamic/respiratory stability of this anaesthetic technique. The secondary outcomes included patient satisfaction and surgeon comfort regarding the anaesthesia technique and postoperative events (nausea and vomiting, urinary retention, postdural puncture headache).ResultsSurgery was performed successfully in 61 patients under TSA (90% female, 53±13 years old). Intraoperative pain scores were low, with a median Numeric Rating Scale score of 0 (IQR 0–2). Surgeon and patient satisfaction scores were excellent. The haemodynamic and respiratory parameters remained stable throughout the surgery. The incidence of postoperative events was low (nausea/vomiting=8%). None of our patients presented with postdural puncture headache or urinary retention.ConclusionTSA could be an effective anaesthetic technique for abdominal surgery in the context of a field hospital or austere environment.
