Objective To retrospectively compare the differences in the surgical efficacy and prognosis of laparoscopic pyeloplasty for hydronephrosis caused by symptomatic versus asymptomatic ureteropelvic junction obstruction (UPJO) in children and determine whether clinical symptoms affect the surgical outcome and prognosis. Methods Children who underwent laparoscopic pyeloplasty in our hospital from January 2018 to December 2022 were retrospectively analyzed. The children were divided into symptomatic and asymptomatic groups according to their main symptoms. The primary outcomes were the surgical success rate, change in renal parenchymal thickness, and change in renal pelvis anteroposterior diameter. The secondary outcomes were postoperative complications, reoperation rate, operative duration, intraoperative blood loss, and drainage tube indwelling time. Results In total, 224 children with UPJO were enrolled; 148 (66.1%) were symptomatic and 76 (33.9%) were asymptomatic. The symptomatic group showed a significantly greater mean change in renal parenchymal thickness, significantly higher surgical success rate, and significantly lower postoperative complication rate. Conclusions In the present study, asymptomatic children had a lower surgical success rate, less postoperative imaging improvement, and more postoperative complications than symptomatic children. The presence or absence of clinical symptoms may affect the surgical outcome and prognosis.

Background Laparoscopic nephron-sparing surgery (NSS) is the standard of care for small renal masses whenever feasible. This study aims to describe a novel laparoscopic aspirator bracket (LAB) and its use in laparoscopic NSS by a simple enucleation technique. Methods Between July 2017 and July 2019, we performed 54 laparoscopic NSS due to renal tumour using an SE technique by either LAB (n = 26) or laparoscopic aspirator (LA, n = 28) in 3 independent medical centres by 6 experienced surgeons. The novel aspirator could aspirate liquid and expose the operative field simultaneously. The details of operative technique are provided in the accompanying video. General characteristics and perioperative data of all patients were collected and retrospectively. Patients were followed up for 12 months after laparoscopic renal tumor simple enucleation (SE) operation. Renal function outcomes and perioperative complications were assessed. Results All 54 patients were successfully operated without conversion to open surgery. The use of the LAB in laparoscopic renal tumor SE operation can shorten the warm ischemia time ([WIT], LAB 12.92 ± 8.38 min vs. LA 17.89 ± 6.73 min) and increase the zero ischemia (ZI) rate (LAB 23.08% vs. LA 3.57%). In the one-year follow-up, the LAB group showed quicker renal function recovery (ipsilateral renal function 3 months after the surgery, LAB 41.67 ± 10.11% vs. LA 36.75 ± 10.30%). The limitations of this study were limited number of patients and retrospective attribute. Conclusion The new LAB could aspirate and expose the operative field with a single instrument. In operations that need to expose and aspirate simultaneously, such as in renal tumor simple enucleation, it could shorten WIT and improve the postoperative renal function recovery.

Pediatric Minimally Invasive Surgery—A Bibliometric Study on 30 Years of Research Activity

Background: Pediatric minimally invasive surgery (MIS) is a standard technique worldwide. We aimed to analyze the research activity in this field. Methods: Articles on pediatric MIS (1991–2020) were analyzed from the Web of Science™ for the total number of publications, citations, journals, and impact factors (IF). Of these, the 50 most cited publications were evaluated in detail and classified according to the level of evidence (i.e., study design) and topic (i.e., surgical procedure). Results: In total, 4464 publications and 53,111 citations from 684 journals on pediatric MIS were identified. The 50 most cited papers were published from 32 institutions in the USA/Canada (n = 28), Europe (n = 19), and Asia (n = 3) in 12 journals. Four authors (USA/Europe) contributed to 26% of the 50 most cited papers as first/senior author. Hot topics were laparoscopic pyeloplasty (n = 9), inguinal hernia repair (n = 7), appendectomy, and pyloromyotomy (n = 4 each). The majority of publications were retrospective studies (n = 33) and case reports (n = 6) (IF 5.2 ± 3.2; impact index 16.5 ± 6.4; citations 125 ± 39.4). They were cited as often as articles with high evidence levels (meta-analyses, n = 2; randomized controlled trials, n = 7; prospective studies, n = 2) (IF 12.9 ± 22.5; impact index 14.0 ± 6.5; citations 125 ± 34.7; p > 0.05). Conclusions: Publications on laparoscopic pyeloplasty, inguinal hernia repair, appendectomy, and pyloromyotomy are cited most often in pediatric MIS. However, the relevant number of studies with strong evidence for the advantages of MIS in pediatric surgery is missing.
