Milwaukee Shoulder Syndrome

Abstract: Milwaukee shoulder syndrome (MSS) is a rare destructive, calcium phosphate crystalline arthropathy. It encompasses an effusion that is noninflammatory with numerous aggregates of calcium hydroxyapatite crystals in the synovial fluid, associated with rotator cuff defects. We describe a patient that presented with recurrent shoulder pain and swelling with characteristic radiographic changes and MSS was confirmed on aspiration of the synovial fluid.

“…7) Its pathology is believed to be the intraarticular calcium hydroxyapatite deposition, which induces the release of lysosomal enzymes, which attack the peri-articular tissues, including the rotator cuff. 8) Unlike monosodium urate and calcium pyrophosphate dehydrate crystals, calcium hydroxyapatite crystals cannot be observed under a plain and polarized microscope. Use of alizarin red stain enables simple and rapid identification of clumps of calcium hydroxyapatite crystals, 9) but we cannot practice this stain.…”

“…A peculiar and very severe condition associated with BCP crystal deposition is the Milwaukee shoulder syndrome (MSS), which can occur in patients between 60 and 90 years of age, due to intra-articular deposition of BCP crystals that release lysosomal enzymes leading to the destruction of articular and peri-articular structures, including the rotator cuff [79]. Apart from shoulders, other joints, including the knees, hips, and/or elbows, might be involved [80].…”

“…However, individual BCP crystals are non-birefringent and are too small to be detected by conventional or polarized light microscopy [81]. Furthermore, despite their tendency to clump, especially when present in large amounts, BCP crystals remain undetectable in the SF [66], which is usually noninflammatory with a low leukocyte count [79]. US, CR, CT, MRI, and DECT can all be helpful for the diagnosis [82].…”

“…Elle débute brutalement par une douleur locale, une impotence fonctionnelle, un gonflement de l'articulation [3]. La ponction retire un liquide synovial typiquement hémorragique, non inflammatoire avec présence de dépôts de phosphates de calcium.…”
