Miliary Sarcoidosis following Miliary Tuberculosis

Abstract: A patient who presented with a miliary radiographic pattern due to tuberculosis and later with a similar miliary pattern due to sarcoidosis is described. The patient, a 47-year-old man, was admitted to the hospital due to coughing, weakness, weight loss and an abnormal chest radiograph with a miliary pattern. A gastric fluid culture was positive for Mycobacterium tuberculosis and he was treated appropriately. He showed complete clinical and radiological remission. One year later he presented with a dry cough a… Show more

“…These include: histopathological appearances of the granulomas [2]; reports of mycobacterial disease either existing coincidentally, succeeding or pre-dating sarcoidosis [59,60]; and the finding of mycobacteria in occasional granulomas of sarcoidosis [61][62][63]. Passage experiments have also suggested that mycobacteria with characteristics of M. tuberculosis may be the incriminating agent [64][65][66][67].…”

“…Some studies [1][2][3][4][5] lend support to this observation. Ning et al, [5] showed the presence of mycobacterial DNA by PCR analysis in 16 of 20 patients with cutaneous lesions of sarcoidosis. The isolation of genetic material from mycobacteria in granulomas of sarcoidosis suggests that the granulomas were initiated by mycobacteria but whether the organisms are viable is still debatable.…”
