Background: effective treatments for migraine aura and related symptoms are not yet well established. In the last years, several herbal and/or nutraceutical preparations have been proposed as potential treatment. We report the results of a retrospective analysis on the synergistic effect of three nutraceutical components (Tanacetum parthenium, Griffonia simpliciofila and Magnesium, Aurastop®) as symptomatic treatment of migraine aura and related symptoms. Method: Forty-nine subjects with headache with aura were recruited from the headache Center of the Istituto Clinico Citta' di Brescia to enter the studied that consist to treat the first 3 aura attacks as usual and the next 3 taking a tablet of Aurastop at the beginning of the aura phenomena. They had to describe aura and headache characteristics of previous three attacks (t1) and the modification of these parameters with the assumption of Aurastop® for the following three attacks (t2). Results: A significant reduction (>50%) in aura duration (t1 = 33.6 ± 10.1 minutes vs. t2 = 9.4 ± 6.2 minutes, p < 0.01 FWER corrected) as well as in overall disability (median [interquartile range]) (t1 = 5[4 -5] vs. t2 = 1[1 -2], p < 0.01 FWER corrected) was evident. Furthermore, modification of aura type as well as a series of parameters more related to headache (number of headache attacks, duration, intensity, utilization of analgesics and response to symptomatic treatment) was influenced by Aurastop® utilization (p < 0.01 FWER corrected). No significant adverse effects were recorded after the assumption of Aurastop®. Conclusions: the combined and synergistic effect of Tanacetum parthenium, Griffonia simpliciofila and Magnesium (Aurastop®) highlights the idea that symptomatic treatment potentially modulating cortical spreading depression could deserve attention to miti-

BackgroundThere is limited literature on prolonged aura (PA - defined as an aura including at least one symptom for > 1 h and < 7d), and there are no prospective studies.The aim of this study is to characterize prospectively the phenotype and prevalence of PA.FindingsTwo hundred and twenty-four patients suffering from migraine with aura were recruited from the Headache Centers of Pavia and Trondheim. Patients prospectively described, on an ad hoc diary, each aura symptom (AS), the duration of AS and headache, and headache features. Seventy-two patients recorded three consecutive auras in their diaries. 19 (26.4%) of patients suffered at least one PA. Out of 216 recorded auras, 38 (17.6%) were PAs. We compared PAs with non-PAs with respect to 20 features; PAs were characterized by a higher number of non-visual symptoms (non-VS) (p < 0.001). No other differences were found. We obtained similar results when we compared auras with at least one symptom with a duration of > 2 h (n = 23) or > 4 h (n = 14) with the the others (n = 193 and n = 202 respectively).ConclusionPAs are quite common. They do not differ from the other auras (even when their duration extends to 2 and/or 4 h) with the exception of a higher number of non-VS.

BackgroundAura symptoms have been reported in up to 23% of cluster headache patients, but it is not known whether clinical characteristics are different in participants with and without aura.MethodsUsing validated web‐based questionnaires we assessed the presence and characteristics of attack‐related aura and other clinical features in 629 subjects available for analysis from an initial cohort of 756 cluster headache subjects. Participants who screened positive for aura were contacted by telephone for confirmation of the ICHD‐III criteria for aura.ResultsTypical aura symptoms before or during cluster headache attacks were found in 44/629 participants (7.0%) mainly involving visual symptoms (61.4%). Except for lower alcohol consumption and higher prevalence of frontal pain in participants with aura, no differences in clinical characteristics were found compared with participants without aura.ConclusionAt least 7.0% of the participants with cluster headache in our large cohort reported typical aura symptoms, which most often involved visual symptoms. No major clinical differences were found between participants with and without aura.

Abstract: We provide data to suggest that symptoms may last longer than one hour in a relevant proportion of auras or migraine with aura patients, and that there is a high variability of scenarios in terms of time relationship among aura symptoms and between aura and headache.
