Micturition Studies and Sexual Function in Operated Hypospadiacs

Abstract: Micturition patterns in 33 males, aged 15 to 34 years, were investigated after hypospadias repair in childhood. Spraying of the urinary stream was the most common complaint (13 patients). Urinary flow was low in five patients as compared with controls, but in only one of these patients was an obstruction found that required treatment. Five patients had persistent curvature of the penis. One of them was unable to perform sexual intercourse.

“…In a more recent study (Aho et al, 2000), despite overall positive “sexual success,” it is noteworthy that 11 (24%) of 46 hypospadiac patients complained of erection problems compared with 3 (7%) of 43 circumcised patients, a statistically significant difference ( P < .01); in addition, 6 hypospadiac patients (13%) presented ejaculation problems. In fact, the data in the literature (Sommerlad, 1975; Kenawi, 1976; Berg et al, 1981; Svensson and Berg, 1983; Bracka, 1989; Aho et al, 2000) show that difficulty in sexual relations, such as pain or dissatisfaction, were reported by 6% to 24% of patients.…”

“…Rarely, a surgical complication such as penile curvature will prevent vaginal penetration [22]. A group of 104 men, aged 24–42 years, matched to controls were evaluated with an unvalidated 15-item questionnaire.…”

“…Studies of adult adjustment have been flawed by the inconsistent application of concepts of gender identity and sexual orientation, such as 'latent homosexuality' and 'uncertain or insecure' gender identity, defined as 'confidence with which the subject assigns himself to the male sex, and the extent to which he feels that he belongs among males' . These studies report varying degrees of psychosexual dysfunction (Sommerlad, 1975;Kipikasa, Longauer, & Urbanova, 1979;Svensson, & Berg, 1983;Bracka, 1989), which have been ascribed to cosmetic and functional abnormalities of the penis, along with lowered post-pubertal testosterone levels.…”
