Microvascular Decompression: Salient Surgical Principles and Technical Nuances

Abstract: Trigeminal neuralgia is a disorder associated with severe episodes of lancinating pain in the distribution of the trigeminal nerve. Previous reports indicate that 80-90% of cases are related to compression of the trigeminal nerve by an adjacent vessel. The majority of patients with trigeminal neuralgia eventually require surgical management in order to achieve remission of symptoms. Surgical options for management include ablative procedures (e.g., radiosurgery, percutaneous radiofrequency lesioning, balloon c… Show more

“…17 Thought to relieve the symptoms of some patients in the early stage, those antiepileptic drugs have many side effects and the patient would resist against it with time. The other choices are those ablative procedures, such as glycerol injections, 18 radiosurgery (gamma knife), 19-23 radiofrequency rhizotomy, [24][25][26][27][28] and so on. Partial sensory rhizotomy has now rarely been performed for TN because of its higher recurrence and intolerable dysesthesia due to nerve damage.…”

“…Cement cranioplasty followed a standardized surgical protocol previously published. 29 After induction of general anesthesia, patients were positioned in the park bench position, and brainstem auditory evoked responses were measured. We performed a curved incision approximately 2 to 3 cm posterior to the pinna.…”

“…To improve postoperative results, some authors have chosen to refine the MVD surgical technique [9][10][11][12][13][14][15] . In this work, we present our experience in the surgical treatment of TN through a minimally-invasive asterional approach for trigeminal vascular decompression.…”
