Microvascular decompression for trigeminal neuralgia using the ‘Stitched Sling Retraction’ technique in recurrent cases after previous microvascular decompression

Abstract: The 'stitched sling retraction' technique shows promising preliminary results in recurrent cases of trigeminal neuralgia after previous microvascular decompression(s). Since it is a 'transposing' technique, it might be associated with less recurrence rates (due to resuming of the neurovascular conflict) than the classic interposing technique, which uses a prosthesis between the offending vessel and the trigeminal nerve.

“…Recurrence can occur because of compression of the nerve from the interposing material placed during surgery or recontact of a vessel with the nerve. A “sling” technique may be used to hold the vessel off the nerve, and it demonstrates promise at lowering recurrence rates despite its technical difficulty [ 87 ]. The most common reported complication of MVD in our study was facial numbness, affecting 4.4% of patients.…”

“…Transposition of the offending vessel with Teflon wool or slings, especially in tortuous NVC, is a useful. [ 170 171 172 173 174 ] Aneurysm clip with or without unabsorbable dural sling can be used. [ 175 176 ]…”

“…A large number of researches have been carried on MVD postoperative recurrence of trigeminal neuralgia, respectively using repeat MVD and percutaneous balloon compression (PBC) [ 2 , 8 , 9 , 19 ]. Therefore, this study is to compare the curative effect of redo MVD and PBC when first MVD is invalid through a prospective cohort study.…”
